What is it?
Pancreatitis is a common and highly unpredictable inflammatory disorder of the pancreas. To understand this condition, it helps to understand the dual role of the pancreas. This small, V-shaped organ nestled alongside the stomach and the first part of the small intestine (the duodenum) has two primary functions. First, it acts as an endocrine gland, producing hormones like insulin to regulate blood sugar. Second, it acts as an exocrine gland, producing powerful digestive enzymes that help break down food.
Under normal circumstances, these digestive enzymes are stored safely inside the pancreas in an inactive state. They are only activated once they travel through the pancreatic duct and enter the small intestine, where they can safely break down proteins, fats, and carbohydrates. In a pet with pancreatitis, this protective mechanism fails. The digestive enzymes activate prematurely while they are still inside the pancreas. This leads to a process called autodigestion, where the pancreas literally begins to digest itself.
This premature activation triggers a cascade of intense inflammation. The disease can present in several ways:
- Acute pancreatitis: A sudden, severe onset of inflammation that can range from mild, self-limiting discomfort to a life-threatening emergency.

How vets diagnose it

It is highly sensitive for typical acute cases because the associated swelling (edema) and death of surrounding fat tissue (peripancreatic fat necrosis) create distinct, visible changes on the screen.
- Feline Pancreatic Lipase Immunoreactivity (fPLI): A highly specific blood test used to detect pancreatic inflammation in cats.
- Pancreatic Biopsy and Histopathology: This remains the gold standard for definitive diagnosis. Biopsies can be taken safely during a minimally invasive laparoscopy using specialized clamshell forceps. However, because anesthesia and surgery carry risks for unstable patients, a biopsy is often bypassed in daily practice unless the diagnosis remains highly uncertain.
- Cytology of Ultrasonography-Guided Fine-Needle Aspirates: Your vet may use ultrasound guidance to safely insert a tiny needle directly into the pancreas to collect cell samples for microscopic evaluation.
- Fluid Analysis of Abdominal Effusion: If inflammation has caused fluid to accumulate in your pet's abdomen, your vet may collect and analyze a sample of this fluid to rule out other causes of peritonitis.
Treatment options
There is no single "cure" for pancreatitis. Instead, veterinary treatment focuses on supporting the pet's body, controlling pain, managing complications, and allowing the pancreas time to heal. The intensity of treatment depends entirely on whether the disease is acute or chronic, and mild or severe.
First-Line Supportive Care
For acute or severe cases, hospitalization is essential. This typically involves:
- Intravenous Fluid Therapy: To restore hydration, correct electrolyte imbalances, and maintain vital blood flow to the inflamed pancreas.
- Pain Management: Pancreatitis is an incredibly painful disease. Vets will use potent, veterinary-specific pain medications to keep your pet comfortable.
- Nutritional Support: Keeping the gut moving is vital. If your pet is too nauseous to eat, your vet may place a temporary feeding tube or prescribe advanced anti-nausea medications.
Second-Line and Adjunctive Therapies
Depending on your pet's specific presentation and the presence of concurrent diseases, your vet may prescribe several targeted therapies:
- Metronidazole: This antibiotic and antiprotozoal medication may be used to address secondary bacterial overgrowth in the intestines or to treat concurrent gastrointestinal issues.
- Cyanocobalamin (Vitamin B12): This nutritional supplement is highly recommended, particularly for cats and dogs with chronic pancreatitis. Chronic gut and pancreatic inflammation frequently impair the body's ability to absorb B12 from food, leading to severe deficiencies that hinder recovery.
- Insulin: If the inflammatory process destroys the endocrine portion of the pancreas (the islet cells that produce insulin), your pet may develop temporary or permanent diabetes mellitus. In these cases, daily insulin therapy is required to manage blood sugar levels.

Prognosis

- Chronic Pancreatitis: Generally carries a low immediate risk of death. However, because it is a progressive, irreversible disease, pets face a high long-term risk of eventually developing exocrine pancreatic insufficiency (EPI) or diabetes mellitus. These secondary conditions require lifelong, daily management.
Prevention
Because most cases of pancreatitis are idiopathic, complete prevention is not always possible. However, you can take several proactive steps to minimize your pet's risk:
- Manage Concurrent Diseases: Work closely with your vet to strictly manage underlying endocrine disorders like hypothyroidism, Cushing's disease, and diabetes mellitus.
- Avoid Known Drug Triggers: If your pet is prescribed potassium bromide, azathioprine, or asparaginase, discuss the risks with your vet and monitor your pet closely for early signs of digestive upset.
- Dietary Vigilance: Avoid feeding your pets high-fat table scraps or sudden changes in diet, which are widely recognized clinical triggers for acute flare-ups, especially in dogs.
