La leucemie touche le chien et le chat, sous forme aigue ou chronique.

Qu'est-ce que c'est ?

Tumeur maligne des precurseurs hematopoietiques de la moelle. Des clones anormaux chassent la production normale (cytopenies). Aigue: blastes immatures, agressive. Chronique: cellules plus matures, plus lente. Lymphoide vs myeloide selon la lignee.

Causes et facteurs de risque

Surtout mutations idiopathiques. Chat: risque FeLV. Aussi radiations, toxiques, chimiotherapie. Certaines races canines surrepresentees.

Signes a surveiller

  • Lethargie, gencives pales, perte de poids, fievre (frequents)
  • Lymphadenopathie, spleno/hepatomegalie, cytopenies (frequentes)
  • Petechies/ecchymoses, signes GI, boiterie, signes neuro (occasionnels)
  • Ictere, lesions oculaires (occasionnels)

Comment les veterinaires diagnostiquent

NFS et frottis; aspiration/biopsie medullaire (gold standard); cytometrie de flux et cytochimie pour la lignee.

Options de traitement

Chimiotherapie selon le type: Chlorambucil pour la LLC; Glucocorticoids; Vincristine; Cyclophosphamide; Cytarabine; Asparaginase; hydroxycarbamide pour la LMC. Support: transfusions, fluides, antibiotiques.

Pronostic

Aigue: mauvais a sombre (semaines a mois). LLC chronique: bon a excellent (annees possibles).

Prevention

Chat: prevenir le FeLV (interieur, tests, vaccination a risque). Chien: bilans et analyses de sang pour detecter les formes chroniques tot.

Quand appeler le veterinaire

Urgent: faiblesse extreme, gencives tres pales, petechies, fievre persistante, collapsus, dyspnee, amaigrissement/anorexie rapides.
