Hvad er det?
Inflammation eller infektion i de nedre luftveje. Alveoler fyldes med væske og inflammationsceller; gasudvekslingen falder. Kan være bakteriel bronkitis eller bronkopneumoni.
Årsager og risikofaktorer
Bakterier (E. coli, Klebsiella, Pasteurella, Pseudomonas, Bordetella, Streptococcus) via inhalation, aspiration eller hæmatogent. Risiko: ganespalte, spiserørssygdom, trængsel, forceret flaskefodring.
Tegn
- Høje åndelyde, krepitationer eller piben
- Hoste (almindelig hos hund, sjælden hos kat)
- Takypnø, åndedrætsarbejde, nedsat appetit, letargi
- Feber, mukopurulent næseflåd, hypoksæmi
Katte hoster sjældent; ofte takypnø eller udstrakt hals.
Diagnose
Auskultation, thoraxrøntgen i 3 projektioner, trakeal-/endotrakeal skylning (guldstandard), oxymetri, blodstatus.
Endotrakeal skylning giver prøver til dyrkning og cytologi.
Behandling
Bredspektrede antibiotika (fx doxycyklin, fluorquinolon, amoxicillin/clavulansyre) mindst 4 uger eller 1–2 uger efter normalt røntgen. Ilt, væske, nebulisering/coupage. Undertryk ikke hosten.
Prognose
God ved tidlig behandling; mere reserveret ved alvorlig aspiration eller resistens. Røntgenkontrol er nødvendig.
Forebyggelse
Luftvejsvacciner, hygiejne, korrekt flaskefodring, tidlig behandling af opkast/dysfagi.
Hvornår man skal ringe til dyrlægen
Akut ved respirationsfrekvens i hvile >40/min, dyspnø, blege/blå slimhinder eller kollaps.
