Was ist das?
Die gesunde Leber entgiftet Darmprodukte wie Ammoniak. Bei Leberinsuffizienz oder portosystemischem Shunt gelangen Neurotoxine ins Gehirn.
Ursachen und Risikofaktoren
Junge Tiere: oft angeborener portosystemischer Shunt (PSS). Ältere: erworbene Lebererkrankung oder Zirrhose. Trigger: proteinreiche Mahlzeiten, Obstipation, GI-Blutung, Entzündung, bestimmte Medikamente.

- Coma

Diagnose

Behandlung
Acute
Lactulose; Propofol/Levetiracetam; Mannitol if cerebral edema. Avoid barbiturates/diazepam or use extreme caution.
Long-term
Antibiotics (Metronidazole, Ampicillin, Neomycin); digestible moderate-protein diet.
Surgery
PSS: e.g. ameroid constrictor.
Prognose
PSS surgery often resolves signs. Medical management variable. End-stage cirrhosis: palliative.
"The treatment of acute encephalopathic crises... is worthwhile because some animals can go on to complete recovery and successful long-term medical management, particularly if the acute crisis was triggered by a definable event (e. g. , acute GI bleeding in a dog with chronic liver disease and portal hypertension)."
Vorbeugung
Angeborene Shunts nicht verhinderbar; verantwortliche Zucht. Bei bekannter Lebererkrankung: Diät und GI-Blutung/Infekte beobachten.
