Pyelonephritis is a serious bacterial kidney infection in dogs and cats that typically travels up from the bladder. Prompt veterinary diagnosis and targeted antibiotic therapy are essential to prevent permanent kidney damage or life-threatening complications.
TL;DR. Nhiễm khuẩn thận nặng, thường lên từ bàng quang; kháng sinh đúng đích và chăm sóc sớm để tránh tổn thương thận vĩnh viễn.
Viêm bể thận gây uể oải và bệnh toàn thân.
Là gì?
Nhiễm trùng bể thận và nhu mô. Thường đi lên từ đường tiết niệu dưới; ít hơn qua máu. Có thể cấp hoặc mạn.
Nguyên nhân và yếu tố nguy cơ
UTI trước đó, dị dạng giải phẫu, nước tiểu loãng, đái tháo đường/Cushing/CKD. Không có khuynh hướng giống ghi nhận.
Dấu hiệu
- Uống/tiểu nhiều, sốt, uể oải
- Đau hông, lưng gù, bỏ ăn, nôn
- Máu trong nước tiểu, mất nước
- Hiếm: vô niệu
Đau hông và tư thế gù là dấu hiệu nhiễm trùng thận cấp.
Chẩn đoán
Công thức/sinh hóa máu, xét nghiệm nước tiểu, cấy nước tiểu hiếu khí định lượng (tiêu chuẩn vàng), siêu âm; đôi khi chọc bể thận.
Siêu âm đánh giá kích thước và cấu trúc thận.
Điều trị
Kháng sinh 4–6 tuần theo cấy (fluoroquinolone như orbifloxacin thường hàng đầu), truyền dịch và hỗ trợ.
Tiên lượng
Tốt nếu cấp và sớm; thận trọng nếu mạn hoặc CKD. Urosepsis hiếm.
Phòng ngừa
Điều trị UTI sớm, tăng uống nước, cấy định kỳ ở bệnh nhân nguy cơ.
Khi nào gọi thú y
Ngay nếu sốt, uể oải nặng, nôn, đau lưng, máu trong nước tiểu hoặc tiểu giảm/ngừng.
