Primary hyperaldosteronism, or Conn's syndrome, is an uncommon endocrine disorder in dogs and cats caused by an adrenal tumor that overproduces aldosterone. This leads to severe potassium depletion and high blood pressure, resulting in weakness and lethargy.

Là gì?
Cường aldosterone nguyên phát (hội chứng Conn): u vỏ thượng thận tiết thừa aldosterone; thận giữ natri và mất kali. Tăng huyết áp do ứ dịch; yếu cơ sâu do hạ kali.
Nguyên nhân và yếu tố nguy cơ
Hầu như luôn là adenoma hoặc adenocarcinoma vỏ thượng thận. Trung niên đến cao tuổi. Doberman Pinscher dễ mắc hơn; nói chung ít gặp.
Dấu hiệu
- Hạ kali máu (chủ yếu)
- Yếu (thường; mèo: gập cổ ventroflexion)
- Uể oải (thường)
- Tăng huyết áp toàn thân (thường)
- Tăng natri máu (thường)
Yếu cơ và uể oải khi kali thấp.
Chẩn đoán
Sinh hóa huyết thanh (Na cao, K thấp), huyết áp, siêu âm bụng tìm khối thượng thận, aldosterone huyết thanh/nước tiểu.
"Excessive secretion of aldosterone causes sodium retention and potassium depletion, which manifest as increased serum sodium (>155 mEq/L) and decreased serum potassium (<3.0 mEq/L) concentrations."
Siêu âm giúp tìm khối thượng thận.
Điều trị
Cắt thượng thận khi khối khu trú và cắt được. Nếu không, bổ sung kali và thuốc hạ áp trọn đời. Phẫu thuật thượng thận rủi ro gần mạch lớn.
Tiên lượng
Rất tốt sau khi cắt bỏ khối lành tính thành công. Thận trọng khi chỉ điều trị nội khoa hoặc bệnh xâm lấn.
Phòng ngừa
Không có phòng ngừa nguyên phát. Xét nghiệm cao tuổi và đo huyết áp giúp phát hiện sớm hơn.
Khi nào gọi
Cấp cứu nếu: yếu sâu đột ngột, mù đột ngột/đồng tử giãn, xẹp.
