Bartonellosis is a common bacterial infection transmitted by fleas and ticks. While many cats carry the bacteria without showing symptoms, it can cause severe heart issues, fever, and inflammation in dogs and vulnerable cats, requiring targeted antibiotic therapy.

Qu'est-ce que c'est ?
Bartonella cible l'endothélium et les hématies. Le chat est souvent un réservoir adapté avec forte bactériémie sans signes ; le chien non adapté peut tomber malade avec une bactériémie faible.
Causes et facteurs de risque
Transmission par puces (fèces dans les griffures) et tiques. Climats chauds et humides. Morsures/griffures (maladie des griffes du chat chez l'homme). Pas de prédisposition de race.
Signes à surveiller
Chien : fièvre, souffle, arythmies ; parfois léthargie, syncope, polyarthrite, épistaxis, thrombocytopénie.
Chat : souvent subclinique ; parfois fièvre, adénopathies, uvéite, gingivite.

L'uvéite est un signe occasionnel chez le chat.
Comment les vétérinaires diagnostiquent
Culture BAPGM + PCR (référence), PCR, hémoculture, sérologie, échocardiographie si endocardite suspectée.

L'échocardiographie recherche des lésions valvulaires.
Options de traitement
Antibiotiques prolongés : doxycycline en première intention ; sinon enrofloxacine ou azithromycine. Endocardite : traitement agressif prolongé.
Pronostic
Chats subcliniques : excellent. Signes inflammatoires : bon sous traitement. Endocardite : réservé à mauvais.
Prévention
Antiparasitaires puces/tiques toute l'année, chats d'intérieur, hygiène après griffures.
Quand appeler le vétérinaire
Fièvre persistante, douleurs articulaires, léthargie. Urgence : collapsus/syncope, dyspnée, saignement non contrôlé ou pétéchies.
