TL;DR. La pancreatitis aguda es una inflamación repentina, dolorosa y potencialmente mortal del páncreas cuando las enzimas digestivas se activan antes de tiempo y el órgano se digiere a sí mismo.

El páncreas está junto al estómago y el intestino delgado.

¿Qué es?

Órgano exocrino y endocrino. En pancreatitis aguda las enzimas se activan dentro del páncreas (autodigestión), con inflamación, necrosis variable y, en casos graves, falla multiorgánica y CID. Es urgencia alta.

Causas y factores de riesgo

A menudo idiopática. Factores: comida muy grasa, obesidad, diabetes/hipotiroidismo/Cushing, trauma o cirugía, fármacos, y en gatos asociación frecuente con EII y hepatopatía (triaditis).

Signos a vigilar

Frecuentes: anorexia, letargo, vómito (más en perros), dolor abdominal (postura de rezo), deshidratación.

Ocasionales: diarrea, fiebre o hipotermia (gatos), ictericia, shock.

Raros: disnea.

Diagnóstico

Ninguna prueba no invasiva es 100% perfecta. PLI sérica, ultrasonido abdominal, radiografías para descartar otras urgencias; biopsia rara en agudos.

Tratamiento

Soporte: fluidos IV, analgesia (a menudo opioides), nutrición precoz (sonda si hace falta), antieméticos y protectores gástricos. En shock, soporte cardiovascular (p. ej. dopamina en infusión).

Pronóstico

Variable. Leve: a menudo bueno. Grave con necrosis o MOF/CID: reservado a malo. Algunos desarrollan pancreatitis crónica, diabetes o IPE.

Prevención

Evitar grasas y basura, peso saludable, chequeos metabólicos, solo fármacos prescritos.

Cuándo llamar al veterinario

Urgencia: vómitos repetidos, dolor abdominal intenso, colapso, ictericia, dificultad respiratoria.

Razas

Mayor riesgo sospechado en schnauzer miniatura (hiperlipidemia), yorkshire terrier y labrador (obesidad).
