Idiopathic epilepsy is a common genetic seizure disorder in dogs and an uncommon one in cats. It is characterized by repeated seizure episodes with no identifiable underlying brain disease or metabolic cause, requiring careful diagnostic rule-outs and lifelong medical management.

Hva er det?
Idiopathic (inherited) epilepsy is recurrent unprovoked seizures with a normal brain between episodes. Neurons fire in a disorganized surge; no structural brain disease or metabolic toxin is found. Common in dogs; uncommon in cats. Diagnosis is by exclusion.
Årsaker og risikofaktorer
Presumed genetic. Distinct from reactive seizures (toxin/metabolic) and structural epilepsy. Onset typically young to middle-aged animals.
Tegn
- Generalized tonic-clonic seizures (common)
- Focal seizures, cluster seizures (occasional)
- Status epilepticus: continuous seizure >5 min or no recovery between seizures (emergency)

Noen raser har milde anfall med huket holdning og stivhet.
Diagnostikk
Physical/neurologic exam between events. Bloodwork and glucose to exclude reactive causes; FeLV/FIV in cats. MRI and CSF when indicated. Diagnosis when extensive evaluation is normal.

Blodprøver utelukker metabolske årsaker.
Behandling
Lifelong anticonvulsants: barbiturates, potassium bromide (generally avoid in cats), zonisamide, levetiracetam; second-line e.g. felbamate.
Prognose
Many pets have good quality of life on therapy. Harder with early onset, large-breed cluster seizures.
Forebygging
Do not breed affected animals or close relatives in predisposed lines.
Når ringe veterinær
Emergency: seizure >5 min, clusters within 24 h, failure to regain consciousness.
