Wat is het?
Extranodaal lymfoom dat nierparenchym infiltreert en vervangt. Bij katten vaak gekoppeld aan de alimentaire vorm. Verstoort filtratie, vochtbalans en erytropoëtine.
Oorzaken en risicofactoren
Multifactorieel; historisch FeLV-verband bij katten. Predispositie: Siamees, Oriental Longhair, Rottweiler, Persian Greyhound. Middelbare tot hogere leeftijd.
Signalen
- Renomegalie (cardinaal)
- Polyurie en polydipsie
- Emaciatie, azotemie, anemie
- Nierfalen (soms)
Toegenomen dorst en gewichtsverlies zijn frequente tekenen van nierdysfunctie.
Diagnostiek
Buikpalpatie, nier-echo, US-geleide FNA (cytologie zeer gevoelig voor lymfoom). Biopsie en immunofenotypering (B vs T) indien nodig.
Echo stuurt diagnose en monstername.
Behandeling
Systemische chemotherapie (unilaterale nefrectomie zeldzaam). Vaak: cytarabine, lomustine. Support: vocht, anti-emetica, eetluststimulantia, nierdieet.
Prognose
Gereserveerd. Zonder behandeling nierfalen in weken. Met chemo vaak tijdelijke remissie en betere levenskwaliteit.
Preventie
Geen specifieke preventie. Vroege opsporing: controles bij risicorassen (bloed, urine, palpatie).
Wanneer de dierenarts bellen
Veranderde dorst/urine, gewichtsverlies of apathie. Spoed bij ernstig braken, totale anorexie, extreme lethargie of anurie.

Siamees, Oriental, Rottweiler en Persian Greyhound: regelmatige controles.
