Wat is het?
Maagdilatatie-volvulus (GDV): de maag vult met gas/vloeistof en draait, sluit inhoud af, comprimeert grote vaten, veroorzaakt shock en maagnecrose. Echte chirurgische spoed.
Oorzaken en risicofactoren
Idiopathisch multifactorieel. Grote/reuzerassen met diepe borst. Risico: één grote maaltijd/dag, snel eten, beweging rond maaltijden, eerstegraads verwant met GDV, leeftijd, stress.
Signalen
- Onproductief kokhalzen (cardinaal)
- Opgezet, strak abdomen; onrust; hypersalivatie
- Tachycardie, bleke slijmvliezen, zwakke pulsen
- Inzakking (soms)
Opgezet strak abdomen met onproductief kokhalzen is klassiek.
Diagnose
Eerst stabiliseren. Rechts laterale buikfoto (dubbele bubbel / omgekeerde C). Lactaattrends, CBC/chemie, stolling.
Röntgen onderscheidt simpele dilatatie van volvulus.
Behandeling
IV-vloeistoffen, antibiotica (bv. Cefazolin), decompressie (orogastrische sonde of trocarisatie), daarna chirurgie: ontwinden, vitaliteit beoordelen, gastropexie. Post-op monitoring; Metoclopramide indien nodig.
Prognose
Ongeveer 85% overleeft bij snelle chirurgie. Slechter bij necrose, DIC, septische peritonitis of ernstige verzwakking.
Preventie
Profylactische gastropexie bij hoogrisicorassen; meerdere kleine maaltijden; beweging rond voeren beperken; voerstress verlagen.
Wanneer de dierenarts bellen
Absolute spoed. Direct naar spoed bij onproductief kokhalzen of plotselinge buikopzetting. Bel vooraf.
Voor specifieke rassen
Hoogste zorg: Deense dog, weimaraner, gordon/ierse setter, standaard poedel, Duitse herder, sint-bernard. Bespreek profylactische gastropexie bij castratie.
