Turtle trapped underwater: drowning rescue and the first hour
Aquatic turtles drown when they cannot reach the surface, usually because they wedged under a dock or were too weak to climb out. Reptiles tolerate low oxygen far better than mammals, so rescue is worth attempting. Aquatic turtles breathe air. They drown for the same reason any air-breathing animal drowns: they could not reach the surface, usually because they were wedged under something or too weak to climb.
The important difference is what follows. Reptiles tolerate a lack of oxygen far better than mammals, so a turtle pulled from the water limp and apparently lifeless is worth treating as an emergency rather than a bereavement, and it needs a vet immediately.

A turtle that appears dead after being trapped underwater may not be.
Reptile tissue survives without oxygen for far longer than mammal tissue, and turtles have been resuscitated after periods that would be hopeless in a dog or a cat. Do not assume death from stillness, from a limp neck or from lack of response.
Get the animal out, keep it warm and head-down, and telephone a veterinary practice with reptile experience immediately. Do not attempt mouth-to-mouth, do not compress the shell forcefully, and do not warm it rapidly with hot water or a direct heat source.

Why turtles get trapped
Almost every case comes down to one of a small number of situations, and all of them are preventable once you know to look.
Under or behind a basking dock. Floating and suction-mounted docks leave a gap between the platform and the glass. A turtle swims into it, turns, and cannot reverse out. This is the most common single cause.
Behind or under decor. Rocks, driftwood and ornaments placed against the glass create wedge-shaped spaces that narrow as the turtle pushes forward.
Filter intakes and equipment. A limb or the head caught against an unguarded intake, or a turtle wedged between a filter body and the glass.
Heaters and their brackets. Turtles push against equipment and can trap a limb behind a bracket, and a turtle trapped against a heater is also being burned.
No usable exit. Deep water with a single haul-out point, or a basking platform the turtle cannot actually climb because the ramp is too steep or too slippery.
Weakness or illness. A turtle with metabolic bone disease, severe respiratory infection, an injury or advanced dehydration may simply lack the strength to reach the surface repeatedly. In these animals the entrapment is a symptom of something else.
Cold water. A turtle in water below the range it needs becomes sluggish and unable to swim properly. This is a common outcome of heater failure.
Another turtle. Aggression and mating behaviour both involve one animal holding another down. In mixed-size groups this drowns the smaller one.
Thread, netting and hair. Anything long in the water can wrap a limb or the neck.
Species mismatch. Musk and mud turtles are bottom-walkers rather than strong swimmers. Housed in a deep tank designed for a slider, they cannot reliably reach the surface, and they drown. Softshells and hatchlings of most species have the same problem.
What to do the moment you find one
Get it out of the water.
Free a wedged turtle by removing the object rather than pulling the animal, if that is possible. A limb pulled against a fixed obstruction is a limb that can be injured or degloved.
Do not swing it.
Swinging a turtle by the tail, shaking it, or spinning it are all recommended in places online. They do not clear the lungs, and they can dislocate the neck or damage internal organs. Turtles have a rigid shell and the organs are suspended inside it, and violent movement transmits directly to them.
Position it head-down and let gravity work.
Hold the turtle with both hands supporting the shell, tilted so that the head is lower than the tail, at an angle rather than fully vertical. Water and fluid will drain from the mouth and nostrils. Hold this for a short period and repeat, rather than holding it inverted for a long time.
Keep it out of water from now on.
A turtle recovering from an immersion must be kept dry. Do not offer it a bowl to sit in, and do not return it to the tank.
Warm it gently.
A turtle that has been in cool water is cold, and cold reptiles cannot mount much of a physiological response. Warm it gradually, using a warm room, a covered heat source at a distance, or a wrapped warm pack, never hot water, never a direct lamp on the animal, and never rapidly.
Do not attempt mouth-to-mouth.
The airway of a turtle is small and easily damaged, the lungs are inside a rigid shell, and blowing air in without a tube risks forcing air into the stomach and worsening compression. Ventilation in a turtle is a veterinary procedure requiring intubation.
Do not press the shell rhythmically as chest compressions.
The lungs cannot be compressed by squeezing a rigid carapace, and moving the limbs in and out repeatedly to pump the lungs is an old technique that risks injury. Move the animal, not the anatomy.
Telephone ahead and travel.
Say that the turtle has been submerged and for roughly how long, and ask for a practice with reptile experience.

Why a turtle that seems fine still needs a vet
This is the part owners most often skip, and it is the part that decides outcomes.
Water that enters the lungs does not simply drain away. It causes inflammation, disrupts the surfaces where gas exchange happens, and introduces bacteria from the tank straight into the lower airway. The resulting pneumonia develops over hours to days, not minutes.
A turtle can therefore be rescued, drain a little water, walk around, eat, and be dead of pneumonia several days later. Veterinary treatment started early, usually with antibiotics and supportive care, changes that outcome. Treatment started when the animal is visibly ill often does not.
The same applies to a turtle rescued from cold water and to one that has been trapped for a period without submersion, because pressure injury, limb damage and dehydration all need assessment.
Telling drowning apart from what looks like it
Respiratory infection with buoyancy change.
A turtle that floats at an angle, tips to one side, cannot stay submerged, and swims lopsided is showing one of the classic signs of pneumonia. Because the lungs lie against the top of the shell and one lung is affected more than the other, the animal lists.
That turtle is at high risk of drowning even though the primary problem is infection, because it cannot control its position in water. Take it out of deep water and get veterinary care the same day.
Normal resting behaviour.
Healthy turtles rest underwater for long periods, wedge themselves comfortably under decor by choice, and sometimes float motionless at the surface. The difference is that a resting turtle responds when disturbed and can dive and surface at will.
Cold torpor.
A turtle in water that is too cool becomes slow and unresponsive, and may sit on the bottom. This looks alarming and is a husbandry emergency rather than a drowning, though it can become one.
Egg binding or constipation.
A female straining, restless, and repeatedly trying to leave the water may be looking for a nesting site rather than in respiratory difficulty. It is still a veterinary matter.

Support the shell with both hands. An open, gaping mouth out of water, or bubbles at the nostrils, are signs to travel on rather than to investigate further at home.
Preventing the next one
Do this audit with the tank full and the lights on, looking at it from the side as a turtle would.
Close every wedge. Look for any space between decor and glass, between the dock and glass, and between equipment and glass that narrows as it goes back. Fill it, remove the object, or reposition it so the gap is open at both ends.
Secure the basking dock. It should be fixed so that it cannot shift, and there should be no swimmable gap underneath it that closes at the far end. Check suction cups regularly, because they fail.
Make the ramp usable. It must be reachable from the water, gently angled, and non-slip. Test it by watching the turtle use it, not by looking at it.
Give more than one exit. A single haul-out point in a deep tank is a single point of failure.
Match depth to species and size. Strong swimmers such as sliders manage depth well. Musk turtles, mud turtles, softshells and hatchlings need shallower water with easy access to the surface everywhere, not just at one end.
Guard the intakes. Filter intakes need a strainer the animal cannot trap a limb in. Heaters need a guard, which also prevents burns.
Remove anything stringy. Netting, plant ties, thread and artificial plants with loose fibres.
Check the heater and the temperature. A failed heater in cold weather produces a torpid turtle. Confirm the correct water temperature range for your species with a reptile veterinarian and check it with a thermometer rather than trusting the dial.
Watch group dynamics. Turtles are not naturally social, and one animal holding another underwater is not play. Separate animals of very different sizes.
What never to do
Do not swing, shake, spin or invert a turtle to clear water.
Do not perform mouth-to-mouth or blow into the nostrils.
Do not compress the shell or pump the limbs as a form of resuscitation.
Do not put a rescued turtle back into water, even shallow water, to see how it is doing.
Do not warm a cold turtle quickly, with hot water, or with a lamp directly on it.
Do not pull a turtle that is wedged. Remove the obstruction instead wherever possible.
Do not decide that a turtle that got up and walked away does not need to be seen.
Do not assume an animal that looks dead is dead. Reptiles survive oxygen deprivation for surprisingly long periods, and that judgement belongs to a vet.
Do not house a musk, mud or softshell turtle, or any hatchling, in deep water designed for a strong swimmer.

When to get help
Go immediately for a turtle that has been submerged and is limp, unresponsive, gaping, blowing bubbles from the nose, or breathing with visible effort.
Go the same day for any turtle that has been trapped underwater at all, even one that seems fully recovered, and for any turtle that floats unevenly, cannot dive, or is at the surface with its mouth open.
Book an appointment for a turtle that has become weak, is struggling to climb onto the basking platform, or has stopped using it, because loss of strength is what turns a normal tank into a drowning risk.

A recovered turtle should breathe quietly with the mouth closed, hold itself level in water, and haul out easily.
Bring how long the animal was trapped or submerged if you know, what it was trapped under or in, the water temperature, the tank depth and layout, whether any fluid drained from the mouth or nose and how much, the species and approximate age, and whether it has had any previous respiratory problems. Expect the vet to want to image the lungs, and expect treatment to continue for some time after the animal appears well.
