Darbepoetin Alfa
Also known as: Aranesp, Nespo
> **Important**: This medication is used to treat severe anemia caused by chronic kidney disease. It helps the body produce more red blood cells but is not a cure for the underlying kidney disease. * **Patience is required**: Therapeutic effects may not be noted for several weeks after starting treatment. Do not expect an immediate increase in energy. * **Commitment**: Treatment requires a significant financial commitment, as the drug is expensive. * **Frequent Monitoring**: Your pet will need regular veterinary visits to check their red blood cell count (PCV), iron levels, and blood pressure. * **Watch for Side Effects**: Contact your veterinarian immediately if you notice signs of high blood pressure (e.g., sudden blindness, behavioral changes, stumbling) or a sudden, severe drop in energy, which could indicate a rare immune reaction where the body stops making red blood cells entirely.
Dosage must be set by your veterinarian for your specific pet. Never give human medication or another pet’s prescription without veterinary guidance.
What it is used for
- Anemia of chronic kidney disease
- Anemia of chronic kidney disease
How it is given
Possible side effects
- Anti-rHuEPO antibody formation leading to pure red blood cell aplasia (PRCA)
- Hypertension
- Seizures
- Iron deficiency (due to rapid RBC production depleting iron stores)
Cautions & contraindications
- Patients with documented anti-rHuEPO antibodies
- Uncontrolled hypertension
- Hypersensitivity to darbepoetin or excipients in the formulation
- Use with extreme caution in patients with pre-existing hypertension; blood pressure must be monitored. Monitor closely for the development of anti-rHuEPO antibodies, which is diagnosed by a sudden, severe drop in PCV, a high myeloid:erythroid ratio on bone marrow cytology, and the exclusion of other causes of anemia. Ensure adequate iron stores prior to and during therapy, as rapid erythropoiesis can quickly deplete iron.
Drug interactions
- Androgens: May increase the sensitivity of erythroid progenitors (interaction has been used for therapeutic effect with epoetin)
- Desmopressin: Can decrease bleeding times when used concurrently with erythropoietin agents
