Wat is het?
Lymfocyten-neoplasie die knopen, milt, lever, mediastinum en andere organen kan aantasten. Juveniel (lymfoblastisch): agressief, vaak mediastinale massa. Adult (lymfocytair): indolenter.
Oorzaken en risicofactoren
Exacte oorzaak onbekend. Leeftijd stuurt de vorm. Geen gedocumenteerde raspredispositie.
Signalen
- Vaak: gewichtsverlies, anorexie, lethargie, splenomegalie, perifere lymfadenopathie
- Soms (kritisch bij jongen): dyspneu, pleurale effusie door mediastinale massa
Lethargie en doffe vacht zijn niet-specifieke tekenen die evaluatie rechtvaardigen.
Diagnostiek
Onderzoek (milt en knopen). Eerst FNA/cytologie; histopathologie (gouden standaard). Röntgen; abdomen-echo om sampling te sturen.
Abdomen-echo helpt organen te beoordelen en biopten te sturen.
Behandeling
Multi-agent chemotherapie (aangepaste COP/CHOP): glucocorticoïden, cyclofosfamide, vincristine, doxorubicine. Freten verdragen chemo vaak goed. Palliatief: alleen glucocorticoïden.
Prognose
Juveniel: slecht (weken-maanden). Adult: vaak 6-18 maanden met chemo of steroïden. Doel: levenskwaliteit.
Preventie
Geen specifieke preventie. Vroege detectie: jaarlijkse checks (jong) en elke 6 maanden (>3 jaar).
Wanneer de dierenarts bellen
Spoed: dyspneu of open-mondademhaling; blauwe/zeer bleke slijmvliezen; collaps; totale anorexie >12-24 u. Snel bezoek bij gewichtsverlies, knobbels of opgezwollen buik.
