Why a fish diet can starve a turtle of a vitamin
Thiamine, vitamin B1, is required for the enzymes that release energy from carbohydrate inside every cell. Nervous tissue has a high, continuous energy demand and almost no capacity to store the vitamin, which is why a shortage shows up as neurological signs before anything else.
Thiaminase is an enzyme found in the tissues of a number of fish species. When a turtle eats one of those fish, the enzyme is released into the gut and cleaves thiamine molecules. It destroys the thiamine that came with the meal, and it destroys thiamine from other foods eaten at the same time.
That is the part owners find counter-intuitive. The problem is not that feeder fish lack nutrients. A whole small fish is a reasonable package of protein, fat and bone-bound calcium. The problem is that certain species carry an enzyme that actively subtracts a vitamin from the rest of the diet.
Which fish contain it is not predictable by looking. It cuts across families and it varies between species that seem similar. Goldfish and rosy red minnows, the two most widely sold as cheap feeders, are among those that contain it.
Two practical consequences follow. First, freezing does not help, because the enzyme survives freezing and thawing. Second, heat does destroy it, which is why a cooked fish is not the same hazard as a raw one, though cooking is not a reason to make fish the staple.
The dose that matters is chronic, not single. One fish will not do this. A fish based diet fed several times a week for months will.

The pattern is consistent. Whole animals are close to adequate because the calcium is in the skeleton and the trace nutrients are in the organs. Pieces of animals are not, because the parts a human would discard are the parts a turtle needs.
What live feeder fish bring into a closed tank
Feeder fish are produced as cheaply as possible, at extremely high stocking density, and they are not raised to the standards applied to fish kept as pets. That production model favours exactly the organisms you do not want in your tank.
Parasites. Fish are the intermediate host for a range of flukes, tapeworm larvae and nematodes. Feeding an infected fish delivers the parasite to its next host directly, and many of these have a life cycle that then completes in the tank.
Bacteria. Aeromonas, Pseudomonas and mycobacteria are common in crowded feeder tanks. A turtle with any break in its skin or shell, or one already stressed by cool water, is vulnerable to them.

Weigh the food and weigh the turtle. Both numbers, tracked monthly, tell you more than any feeding chart.
Water quality. Uneaten fish, and the parts of a fish a turtle tears off and abandons, decompose fast. That raises ammonia, which damages skin and eyes and drives shell infection.
Injury. Fish have spines and bones. Turtles tear rather than chew, and a swallowed spine or an oversized bony fragment can damage the mouth or oesophagus.
Welfare and law. Feeding live vertebrate prey is regulated or prohibited in some countries. Where it is legal it is still hard to justify when the same nutrition can be provided by a dead or frozen whole item.
The turtle that will only eat fish
Turtles form strong food preferences, and a young turtle fed live fish learns that food is a thing that moves and can be chased. Pellets and greens do not move, and a turtle that has learned to hunt often ignores them completely.
This is a training problem, not a taste problem, and the failure mode is predictable: the owner changes the diet abruptly, the turtle refuses everything for days, the owner panics and goes back to fish, and the animal learns that holding out works.
The way through it is gradual. Reduce the frequency of the preferred food rather than removing it, offer the new food first while the turtle is hungriest, and put it in the water at the same time and place each day so the turtle associates that spot with food. Movement helps: greens drifting in the current, or a pellet dropped so it sinks past the turtle, both trigger a feeding response better than food sitting still on the bottom.
Check the water temperature before blaming the food. A turtle below its preferred temperature range digests poorly and loses appetite, and no amount of diet engineering fixes a cold tank.
Weigh the turtle before you start and weekly through the change. A turtle that is losing weight during a diet change needs a vet rather than more patience.
How much animal protein, and how that changes
The single biggest variable is species, and owners frequently apply advice written for one turtle to a different one.
Sliders, cooters and painted turtles are omnivores that shift towards plant material as they mature. A hatchling eats mostly animal prey. An adult should be taking a substantial proportion of plants, and continuing to feed an adult like a hatchling is the standard route to obesity and shell deformity.
Musk, mud and snapping turtles stay far more carnivorous throughout life, so the same plant heavy diet that suits an adult slider does not suit them.
Softshell turtles are strongly carnivorous and also have delicate skin, which makes water quality after a protein meal more critical, not less.
Map turtles specialise in hard shelled invertebrates, and a diet without any of that is missing both the nutrition and the jaw work.
Box turtles are omnivores that take a wide range of invertebrates, fruit and plants, and they are terrestrial, which changes how food is presented entirely.
Tortoises are not in this article. They are herbivores. Animal protein given to a tortoise contributes to shell pyramiding and to renal disease, and the fact that a tortoise will eat it does not make it food.
Age matters as much as species. Growing turtles need proportionally more protein, adults need less, and the commonest keeper error is failing to make that transition. Females producing eggs have a genuinely increased demand for calcium and energy, which is a reason to review the diet with a vet rather than to add more fish.
Signs the protein side of the diet has gone wrong
Early.
Weight climbing faster than the shell length, so the limbs and neck look packed into the shell and cannot fully retract. Reduced interest in greens. Water fouling faster than it used to between changes.
Established.
Shell growth that is lumpy rather than smooth, with raised scutes. Soft areas on the shell or a jaw that feels flexible, both pointing to calcium and phosphorus imbalance. Swollen eyelids or a thick discharge at the eyes, which points at vitamin A. A turtle that basks constantly and moves little.
Late.
Neurological signs: weakness in the limbs, a head tilt, tremor, circling, a stretched rigid neck, or seizures. These are the thiamine picture, and they do not resolve by themselves.
At the other end, a turtle fed only fish can also arrive underweight, because a diet that is monotonous and hard to digest at the tank's temperature is not the same as a diet that is adequate.

Species decides the ratio. A box turtle, a slider and a musk turtle should not be eating from the same recipe.
The look-alikes for a wobbling turtle
Neurological signs have several causes and the diet is only one of them. Naming the alternatives helps you give the vet the right history.

The one that matters most for owners to know about is that ivermectin, a common parasite treatment in other species, is dangerous in chelonians. Never accept a parasite treatment for a turtle without confirming the vet knows it is a turtle.
Safer protein, and how to build the change

The target: a settled turtle after a varied meal, with smooth scute growth and limbs that still retract fully.
What the vet will ask for
Bring the diet history in detail. It is the part of the consultation that changes the diagnosis.
Have ready: exactly what has been fed and how often, over how many months; where feeder animals were bought; whether any supplement is used and how it is applied; the water temperature and basking temperature measured with a probe rather than a stick-on strip; the UVB lamp type and when it was last replaced; the weight history; and whether other animals share the tank.
If there are neurological signs, note when they started, whether they are constant or come and go, whether one side is affected more than the other, and take a video. A short clip of the turtle swimming is worth more than a description.
Expect the vet to want to examine the mouth and ears, to consider imaging if the shell or bones are involved, and to ask for a faecal sample if live prey has been fed.
What never to do
Do not build a diet on feeder goldfish or minnows because a turtle enjoys them. Enthusiasm is not a nutritional endorsement.
Do not feed wild caught fish, frogs or tadpoles. They add parasites, and amphibians in particular can carry toxins in the skin.
Do not feed raw mince, chicken or other muscle meat as a staple. The calcium to phosphorus ratio is badly wrong and the result in a growing turtle is metabolic bone disease.
Do not give a vitamin injection or an over the counter supplement on your own initiative to correct a suspected deficiency. Fat soluble vitamins accumulate, and vitamin A overdose produces its own severe skin disease.
Do not thaw frozen fish and assume freezing has made it safe. Freezing kills some parasites but does nothing to thiaminase.
Do not change the whole diet overnight in a turtle that is already thin or unwell. Fix the medical problem first, then the diet.

Get help sooner if food refusal is combined with anything else, particularly reduced basking, listing in the water, bubbles at the nose or a swelling on the head. Those point to problems that a diet change will not touch.
Ask specifically for a vet who sees chelonians. Diet related disease in turtles is diagnosed by putting husbandry, feeding history, weight and radiographs together, and that is not a five minute consultation.
