Turtle and tortoise calcium, UVB and metabolic bone disease
Metabolic bone disease happens because calcium is eaten but not absorbed, and absorption depends on vitamin D3 made in the skin under ultraviolet B light. This guide explains why glass blocks UVB, why a lit lamp can be producing nothing useful, how temperature and the calcium to phosphorus balance fit in, what the disease looks like at each stage, and why a normal blood calcium result does not rule it out.

Quick answer
Metabolic bone disease is the most common serious husbandry problem in pet turtles and tortoises, and almost every case comes from the same place: calcium is being eaten but not absorbed. Absorption depends on vitamin D3, and reptiles make D3 in the skin using ultraviolet B light. No usable UVB means no D3, and no D3 means the calcium in the food passes straight through.
When absorption fails, the body keeps blood calcium normal by taking it out of the skeleton and the shell. That is why an affected animal often has completely normal blood calcium on a test while its shell is going soft. The damage shows in the bones long before it shows in the blood.
Calcium in the bowl is only useful if the animal has the ultraviolet light and the body temperature to absorb it.
- Glass and most plastics block UVB. A turtle sitting by a sunny window is getting no useful ultraviolet light at all.
- UVB lamps stop producing useful output long before they stop producing visible light. They are replaced on a schedule, not when they fail.
- Warmth is part of the same system. A reptile below its preferred temperature does not process calcium properly however good the lighting is.
- A normal blood calcium result does not rule this out. The skeleton is sacrificed to keep the blood normal.
- Shell deformity that has already formed is permanent. Bone can remineralise; shape does not go back.

Do not give extra calcium or vitamin D3 supplements without veterinary advice.
Oversupplementing vitamin D3 causes calcium to be deposited in soft tissue, including the kidneys and blood vessels, and that damage is not reversible. The correct response to suspected bone disease is to fix the ultraviolet light, the temperature and the diet, and to have the animal assessed. Powders and drops given on guesswork can turn one serious problem into two.

How the system actually works
Three things have to line up for a turtle or tortoise to use calcium.
Ultraviolet B light. Reptile skin contains a precursor that is converted to vitamin D3 when it is struck by ultraviolet light in a narrow band. That D3 is then processed by the liver and kidney into the active form that allows calcium to be absorbed from the gut.
The practical points follow directly. Ordinary window glass blocks the relevant wavelengths almost completely, so a basking spot in a sunny room provides warmth and visible light and no D3 at all. The same applies to acrylic tank lids and most plastic sheeting. Fine metal mesh between the lamp and the animal absorbs a substantial share of the output. And a lamp's ultraviolet output declines steadily with hours of use while the visible light stays the same, so a lamp that looks perfectly fine can be delivering nothing useful.
Temperature. Reptiles are ectotherms and their biochemistry runs at the temperature of the animal. Digestion, D3 conversion and bone metabolism all require the animal to reach its preferred body temperature by basking. A turtle in cool water with a cool basking platform will not use calcium properly even under a brand new lamp.
The diet itself. Calcium has to be present in the food, and it has to be present in a reasonable balance against phosphorus. Phosphorus competes with calcium, so a diet that is rich in phosphorus and poor in calcium produces a deficiency even when calcium intake looks adequate on paper. Meat, most insects, many pellets and most fruits are all phosphorus-heavy.
When any of the three fails, the parathyroid gland responds by mobilising calcium from bone. Blood calcium stays within normal limits, the animal seems well for a while, and the skeleton and shell quietly demineralise.
What it looks like, stage by stage
Early.
Subtle and easy to miss. Reduced appetite, less activity, less time basking, slower growth. The plastron may just begin to give slightly under gentle pressure from a thumb. In aquatic species the animal may start swimming with an odd tilt because it is weaker on one side.
Established.
The shell becomes soft or rubbery and can be indented. Marginal scutes at the edge of the carapace curl upwards. Growth becomes uneven, with lumpy raised scutes in tortoises. The jaw softens, the beak overgrows and no longer meets properly, and the animal struggles to bite pieces off food. Limbs swell where bone has been replaced by fibrous tissue. Walking becomes a drag rather than a lift, with the plastron scraping the ground.
Late.
Bones fracture under normal loads, sometimes with no obvious trauma. The spine and shell deform permanently. Blood calcium finally falls, producing muscle tremors, twitching and seizures. Gravid females become unable to pass eggs because the muscular effort required depends on calcium. At this point the animal is critically ill.
Timing varies enormously. A hatchling growing fast under bad husbandry can reach a serious state within months. A large adult tortoise with reserves can take years, and is often presented for something else entirely.

Weighing regularly and writing it down catches the slow decline that a weekly glance never will.

Any localised change, any discharge and any smell moves the diagnosis away from bone disease and towards infection or injury.
Getting the husbandry right
Ultraviolet light.
Use a lamp designed to produce UVB for reptiles, mounted so nothing blocks it. Not through glass, not through acrylic, not through a closed lid.
Mount it at the distance the manufacturer specifies for that product, measured to the animal's shell at the basking site, and take mesh into account if the enclosure has it.
Write the installation date on the lamp with a marker and replace it on the manufacturer's schedule regardless of whether it still lights.
Real unfiltered sunshine outdoors, in a secure shaded enclosure with the option to get out of the sun, is excellent when the weather allows. Never leave a chelonian in an enclosure with no shade, and never in a glass tank in direct sun, where it will overheat.
Heat.
Provide a dry basking site under a heat source, with a temperature gradient across the enclosure. Aquatic turtles need to be able to leave the water completely and dry off. Measure the basking surface with a probe or an infrared thermometer rather than trusting a stick-on dial, and check the correct range for your specific species.
Diet.
Match it to the species and its life stage. Many aquatic turtles are more carnivorous as juveniles and shift towards plant material as adults. Mediterranean tortoises are grazers that need high-fibre weeds, grasses and leaves rather than salad or fruit.
Build the diet on calcium-adequate items. Dark leafy greens, appropriate weeds, and for aquatic species a good quality complete pellet as part of the diet rather than all of it.
Avoid making a staple out of spinach, chard, beet greens or rhubarb. Their oxalate content binds calcium and prevents it being absorbed.
Avoid fruit as a routine item for most species, and avoid dog and cat food entirely.
Offer cuttlefish bone. Tortoises will gnaw it, and it can be floated in an aquatic setup for turtles to bite at. It is a source the animal takes voluntarily.
If a supplement powder is used, get the type and frequency from a vet rather than the packaging. Whether it should contain D3 depends entirely on the ultraviolet provision.

Leafy, fibrous, varied plant material for a grazing species. A diet of one item, however good that item is, is where deficiency starts.
What commonly goes wrong
Assuming a window is enough. It is the single most common reason a well-cared-for animal develops bone disease.
Never changing the lamp. The second most common. A lamp that has been running for years is a heat and light source, not a UVB source.
Buying a combined bulb and assuming it does everything. Some products marketed as full spectrum produce very little useful UVB. Check what the product actually specifies.
Putting mesh, glass or a lid between the lamp and the animal.
Getting the distance wrong. Too far and the output is negligible; too close and it can cause eye and skin damage. Follow the specification for that lamp.
Feeding one thing. Lettuce-only, pellet-only and shrimp-only diets all produce the same outcome by different routes.
Supplementing instead of fixing the light. Calcium powder does not compensate for an animal that cannot absorb calcium.
Treating a soft shell as cosmetic. It is a skeletal disease with a soft shell as one visible sign.
Missing the temperature side. A perfect lamp over a cold enclosure does not work.
What never to do
Do not rely on sunlight through a window.
Do not keep a UVB lamp past its stated life because it still lights up.
Do not place glass, acrylic or a solid lid between the lamp and the animal.
Do not give vitamin D3 supplements without veterinary direction.
Do not feed dog food, cat food, bread or processed human food.
Do not build a diet on spinach, chard or fruit.
Do not press hard on a shell to test it. Gentle thumb pressure on the plastron is enough, and a demineralised shell can be damaged.
Do not put a chelonian outdoors without shade, water and secure escape-proof boundaries.
Do not assume a normal blood calcium result means the skeleton is fine.

When to get help
Go the same day for tremors, twitching, an animal that cannot right itself or cannot lift its body to walk, a gravid female that has stopped eating or is straining, or any turtle with open-mouthed breathing.

A firm shell, a beak that meets properly and a body lifted clear of the ground when walking are what you are checking for.
Book an appointment promptly for a shell that gives under gentle pressure, a soft or misaligned jaw, an overgrown beak, swollen limbs, uneven or lumpy shell growth, or a chelonian that has never had a UVB source.
Bring the exact species and age, the make and model of the lamp and when it was installed, the mounting distance and whether mesh or glass sits in between, measured basking and water temperatures and how you measured them, a full list of what the animal eats in a week, any supplement with its packaging, the weight record, and photographs of the whole enclosure as well as of the animal. Expect a reptile vet to want radiographs, since bone density and folding fractures are visible there and are not visible on the outside, and to treat the husbandry as the main part of the prescription.
