The underweight lizard: body condition and safe refeeding
A thin lizard is a symptom, and feeding it more without finding the cause usually fails. This covers how to read body condition in dragons, geckos, chameleons and monitors, the split between not eating and eating while still losing, the fixed order of recovery, why refeeding a starved reptile too fast is dangerous, and how the plan differs by age.

Quick answer
Putting more food in the bowl is the last step, not the first. A cold lizard cannot digest what it eats, and a dehydrated one should not be loaded with protein.
A thin lizard is a symptom, and feeding it more without finding the cause usually fails. Weight loss in a lizard falls into two very different groups: the animal that is not eating, and the animal that is eating and still losing. The second group is the one that needs a vet soonest.
Recovery has a fixed order. Fix temperature, then hydration, then the cause, then calories. Reverse that order and you get undigested food fermenting in a cold gut, which makes a thin lizard sicker rather than heavier.
- Weigh on a gram scale, weekly, at the same point in the feeding cycle, and write it down. Impressions are not a weight trend.
- A lizard eating well while losing weight needs a faecal test and a veterinary examination, not a bigger portion.
- Verify basking surface and cool end temperatures before changing anything about the diet.
- Never force feed a cold, dehydrated or collapsing lizard. It causes regurgitation and aspiration.
- Severe emaciation is a veterinary case. Reintroducing food too fast to a starved animal is dangerous in itself.

After prolonged starvation the body has shifted to running on fat and protein. Reintroducing food triggers a metabolic switch that pulls phosphate, potassium and magnesium out of the blood and into cells, and the resulting deficits affect the heart and the muscles. This is refeeding syndrome, and it is why an emaciated animal is refed slowly and under supervision rather than given a large meal.
If your lizard has lost a large proportion of its body mass, has a spine and hip bones that stand out sharply, or has not eaten for a long time, the first meal is a veterinary decision. Get it seen before you feed it up.

Judging condition, species by species

Weigh the animal, not just the food. A scale that reads in single grams is the minimum, and tenths of a gram is better for small geckos.
Weight on its own is close to meaningless without a trend and without a look at the body. Read both.
Bearded dragons and other agamids. Look at the tail base and hips from above. A well conditioned dragon has a rounded tail base that tapers smoothly and hips that do not stand out as points. As condition falls, the pelvis becomes prominent, the flanks hollow between the ribs and hips, and the skin over the sides starts to look loose and folded.
Leopard geckos and other fat-tailed geckos. The tail is the fat store and the gauge. In a healthy adult it is broad, at least as wide as the neck, and firm. A tail narrowing toward the base while the animal is still eating is the classic warning picture and should prompt a specific conversation about Cryptosporidium.
Crested geckos and similar arboreal geckos. Hip bones and the ridge of the spine become visible early, and the body loses its filled-out look between the shoulders and hips. Be careful not to confuse a soft or misaligned jaw, which is metabolic bone disease, with thinness.
Chameleons. They store very little fat anywhere, so they deteriorate quickly. Watch the ridge along the spine, the prominence of the casque, and the shape of the eye turrets. Sunken turrets mean dehydration, which is often the more urgent half of the problem.
Uromastyx. The tail base is the store, and their droppings are normally very dry. Thin plus loose output in one of these is a strong signal.
Monitors and tegus. Muscle over the spine and the width of the tail base. In larger lizards, loss of muscle mass along the back is easier to see than loss of fat.
Take a photograph from directly above and one from the side, on the same surface, on the same day each month. Human memory of an animal's outline is unreliable, and a set of dated photographs makes a slow change obvious.
Why the lizard is losing
Split the problem in two, because the branches lead to different places.
Group one: not eating enough.
Temperature is the most common cause and the easiest to miss. A lizard that cannot reach its preferred body temperature will reduce or stop feeding, because it cannot digest.
Then: shedding, which suppresses appetite in many species. Seasonal cooling and brumation in species that do it. Gravidity, when a female stops eating as the eggs take up space. Recent move, new enclosure, a change of position in the room, or a new person or pet in view of the enclosure.
Then the medical ones: mouth infection, jaw pain, retained shed constricting toes or the tail, respiratory infection, impaction, metabolic bone disease, kidney disease, gout, and simply pain from an injury nobody saw.
Also the management ones, which owners rarely suspect: prey too large or too intimidating, feeder insects that escaped and are now hiding in the substrate, salad offered at the wrong time of day for a species that feeds in the morning, or a lizard that has learned that a hand in the enclosure means being picked up.
Group two: eating and still losing.
This is the group that needs testing rather than more food. It points at parasites, particularly coccidia in bearded dragons and Cryptosporidium in leopard geckos, at maldigestion from chronic low temperatures, at organ disease including kidney and liver problems, at neoplasia in older animals, and at food that does not contain what the animal needs.
The last one is common and fixable. Feeder insects contain roughly what they were fed. Insects kept in a tub of bran for a week and then offered are close to empty calories, and a lizard can eat a great many of them and stay thin.
Group three, which is really a management problem: competition.
In any cohabited enclosure, one animal takes the best basking spot and eats first. The other one is thin. Separating them is diagnostic as well as therapeutic, because it is the only way to know whose droppings and whose intake you are measuring.
The recovery sequence
Do these in order. Skipping to step four is the standard mistake.
One: correct the thermal gradient.
Measure the basking surface and the cool end with a probe or infrared thermometer, at the height the animal actually sits. Correct anything outside the species range. Check UVB lamp type, age, distance and whether mesh sits between the lamp and the animal, because a lizard that cannot process calcium will not thrive on any diet.
A thin lizard also needs the option to be warm for longer. Check that the basking site is genuinely reachable for an animal that has lost strength.
Two: restore hydration.
Reptiles concentrate their waste as solid urate, and a dehydrated lizard has slower gut transit, thicker gut contents and stressed kidneys. Loading a dehydrated animal with protein makes the kidney problem worse.
Match the method to how the species drinks. Bearded dragons and many agamids use a bowl. Chameleons take moving droplets and generally ignore standing water. Crested geckos drink from misted surfaces. A shallow, warm, supervised soak helps some species; never leave a weak animal unattended in water and never soak a collapsing one without veterinary advice.
Three: find the cause.
Faecal sample, veterinary examination, and blood work where indicated. Do not treat on suspicion, and in particular do not give an over-the-counter wormer, which will not touch the protozoa that cause most of these cases.
Four: rebuild calories, carefully.
Frequency before volume. Several smaller feeds spaced out are digested better than one large meal in an animal whose gut has been under-used.
Quality before quantity. Gut-load feeder insects on a proper diet for a day or two before offering them, and dust according to your vet's or your species guide's advice. Black soldier fly larvae are useful because their calcium content is naturally favourable. Dubia roaches and crickets are reasonable staples. Waxworms, butterworms and superworms are calorie dense but fat heavy with a poor calcium to phosphorus ratio, so they are a short-term tool rather than a plan.
For herbivorous and omnivorous species, calorie density comes from the right vegetables rather than from fruit. Fruit adds water and sugar, and too much of it causes the loose droppings that make everything worse.
Do not routinely feed pinkie mice to small insectivorous geckos to fatten them. It is a very fat-heavy meal for an animal built for insects, and it loads the liver.
Five: measure the response.
Weigh weekly, on the same scale, at the same point in the feeding cycle, ideally before a meal rather than after. Photograph monthly. Expect slow gain: reptile metabolism is slow and a recovery curve measured in weeks is normal.
If there is no upward trend after two to three weeks of corrected husbandry and a good diet, stop increasing food and go back to the diagnosis.
What changes with age

Chopped to a size the animal can swallow whole, offered at the time of day the species feeds, and within reach of the basking site so digestion can start.
Hatchlings and juveniles.
They grow continuously and have almost no reserve. Failure to gain is as serious as active loss. Any period without food is significant, and a juvenile that goes off food needs an appointment rather than a week of watching. Growth also means they must eat frequently, so a schedule built for an adult will starve a youngster.
Juveniles are also where coccidia and adenovirus problems concentrate, and where a poor start in a mixed collection shows up.
Subadults.
Growth slows and the animal's needs change. Two mirror-image mistakes happen here. Some owners keep feeding at juvenile frequency and produce an obese subadult. Others cut back too early and produce a thin one. Judge by the body and the weight trend, not by the calendar.
Adults.
Maintenance, with seasonal variation that is normal in many species. Adult females lose condition after producing eggs and need a deliberate recovery period with good calcium provision. A previously stable adult that starts losing weight without a season or a clutch to explain it is the classic presentation for parasites or organ disease.
Older lizards.
Muscle mass falls with age, jaw and dental problems make eating harder, kidney disease becomes more likely, and thermoregulation becomes less efficient. Gains come more slowly and the target may be stability rather than a return to a previous weight. Softer or smaller food items, a bowl closer to the basking site, and more frequent smaller meals all help.
Assist feeding, and when it is not your job
Assist feeding means placing food in the mouth of an animal that will not eat voluntarily. It is sometimes necessary and it is easy to do harm with it.
The rules that matter:
Never assist feed a cold lizard. Food placed in a gut that is not moving ferments, and the animal may regurgitate it.
Never assist feed a dehydrated one. Fluids come first, and in a badly dehydrated reptile that means fluids given by a vet.
Never assist feed a collapsing or unresponsive animal. Aspiration into the lungs is a realistic and often fatal outcome, and reptiles clear their airways poorly.
Use the right formula. Critical care products for herbivores and for carnivores are different, and giving a herbivorous lizard a carnivore formula, or the reverse, does harm.
Get the technique shown to you once by a vet or a nurse rather than learned from a video. The volume, the frequency and the way the mouth is opened all matter, and jaws in a lizard with metabolic bone disease break easily.
The failure modes of the usual advice
"Feed it more." Without heat, the food is not digested. Without a diagnosis, the parasite keeps taking what you add. Without hydration, the extra protein burdens the kidneys.
"Fatten it up with waxworms." It works as a number on a scale and produces a lizard with a poor mineral balance and a fatty liver. Use high-fat feeders as a short bridge only, on veterinary advice, and never as the plan.
"It is just brumating." Brumation is a controlled seasonal process in species that do it, entered from a healthy weight, with prior veterinary clearance. Weight loss with lethargy at the wrong time of year, or in a species that does not brumate, is illness. Cooling a sick lizard for brumation is dangerous.
"Deworm it and see." Blind worming misses coccidia entirely, misses Cryptosporidium entirely, and wastes the diagnostic window. Several wormers are dangerous in some reptile groups.
"Give it a probiotic and a vitamin boost." Reasonable supporting measures. Not a substitute for finding out why the animal is thin, and over-supplementing fat-soluble vitamins causes its own problems.
"He is just a small one." A lizard can be genuinely small for its species and still be in good condition. The distinction is body condition and weight trend, not absolute weight.
What the vet will ask for
What never to do
Do not increase food before the temperatures are verified and corrected.
Do not force feed a cold, dehydrated or unresponsive lizard.
Do not give an over-the-counter wormer, and never a product formulated for livestock or dogs.
Do not use fruit or high-fat feeders as the main route to weight gain.
Do not cool a thin or unwell lizard for brumation.
Do not weigh on a scale that reads in five-gram steps for a small gecko. The change you need to detect is smaller than the resolution.
Do not keep a thin lizard in a shared enclosure while you treat it. You cannot control intake, you cannot attribute droppings, and the competition may be the cause.
Do not accept a stable weight as success if the animal started severely underweight. Stability at a low weight is a plateau, and the plateau usually means something is still wrong.

When to get help

The target is a lizard with a filled outline, a rounded tail base, a steady weekly weight and normal droppings, not simply a higher number on the scale.
Same-day reptile veterinary care for a lizard that is emaciated and also dehydrated, one that has collapsed or cannot right itself, one with a jaw or limb that looks deformed or is being carried oddly, or a juvenile that is thin and not eating.
This week for progressive weight loss over several weeks, any weight loss with a normal appetite, failure to gain in a growing animal, a shrinking tail in a fat-tailed gecko, or a lizard that has not eaten for longer than is normal for its species and season.
Take the numbers, the photographs, the food and the supplements. In a thin lizard, the diagnosis usually turns out to be somewhere between the thermometer and the faecal sample, and the sooner both are in front of a vet, the better this goes.
