Feather Destructive Behaviour: The Medical Causes to Exclude First
Feather plucking is usually treated as a psychological problem, but a large share of birds that damage their feathers have a physical reason for it. This guide leads with the differentials that need excluding first, including skin disease, pain, systemic and reproductive illness, giardia in cockatiels, heavy metal exposure and viral feather disease, sets out the workup that answers them, and explains what owners must never do while it is under way.

Quick answer
Feather destructive behaviour is treated online as a psychological problem. It is not, or at least not until a vet has ruled out the physical causes, and a substantial share of birds that pull, chew, or barber their feathers turn out to have a medical reason for doing it.
That order matters because it is a race. Enrichment, more attention, and a new toy rotation cost weeks, and during those weeks a treatable skin infection, a painful joint, a parasite, or heavy metal in the gut carries on doing damage. Follicles that are damaged repeatedly may never grow a normal feather again.
The first clue is usually found by parting the plumage: chewed shafts, snapped barbs, or skin visible where feathers should be.

Why the medical workup comes first
A bird has one tool for dealing with an unpleasant sensation anywhere on its body, and that tool is its beak.
Itch, pain, tingling from a nerve, discomfort from an organ under the body wall, or the irritation of an abnormal feather trying to emerge all produce the same visible result: repeated attention to one area, then damaged feathers, then damaged skin.
Because the behaviour is identical whatever started it, watching the bird cannot tell you the cause. Neither can the internet, and neither can the pattern of damage on its own, though the pattern narrows the list usefully.
There is a second reason for the order. Feather destructive behaviour becomes self-sustaining. A bird that started because of a treatable itch can carry on after the itch is fixed, simply because the behaviour has become established. The earlier the medical cause is found and removed, the less established that habit is.
So the sequence is: full veterinary workup, treat what is found, then work on the behaviour and the environment. The behavioural side of this is covered separately in the over-preening and feather destructive behaviour guide, and it is genuinely important. It is just second.
The medical differentials worth excluding
Skin disease.
Bacterial or fungal infection of the skin and feather follicles, dermatitis, and dry, flaking, itchy skin are all direct causes. Homes heated through winter run very dry, and low humidity alone makes many parrots itchy. Skin cytology and sometimes a small skin and follicle biopsy answer this quickly, and a biopsy is more informative than most owners expect.
Pain anywhere the bird can reach.
Birds pick over what hurts. Old fractures, arthritis, gout in the feet and legs, bumblefoot, an abdominal mass, air sac disease, and pain from the reproductive tract all produce localised picking. Picking that concentrates over one spot, particularly over the flank, the hip, the wing web, or the lower abdomen, deserves imaging rather than a toy.
Reproductive and hormonal disease.
Chronic egg laying, oviduct disease, and prolonged hormonal condition are common in pet hens, and picking over the vent and lower abdomen is a recognised pattern. Chronic hormonal state also changes skin and feather quality in both sexes.
Systemic illness.
Liver disease, kidney disease, and chronic infection all degrade feather quality and make birds uncomfortable. A bird with a chronic hepatopathy often has dull, poorly formed, oddly coloured feathers before anything else is noticed. Blood work is not an optional extra in this workup.
Nutritional disease.
Long-term seed diets leave birds short of vitamin A and of the amino acids feathers are built from. That produces poor feather structure, dry skin, and a bird that is both itchy and unable to grow a good replacement. Diet correction is slow and it is part of the fix rather than the whole of it.
Giardia, particularly in cockatiels.
This is the one most often missed. Giardia is a protozoal parasite of the gut, and in cockatiels it is classically linked to intense irritation, screaming while picking, and feather damage under the wings and along the flanks and thighs. Affected birds may also have loose or bulky droppings and poor weight gain, though some have unremarkable droppings.
Testing is faecal, and the catch is that shedding is intermittent, so a single negative sample does not clear the bird. Ask your vet about repeat or pooled samples across several days. It is treatable, which makes missing it particularly painful.
Other parasites, with a caution.
Feather and quill mites and lice are worth considering in aviary birds, in finches and canaries, and in birds with outdoor exposure. They are genuinely uncommon in a single indoor parrot, which is why reflexive mite treatment is a mistake rather than a shortcut.
Heavy metals.
Zinc and lead are absorbed from swallowed metal and cause multi-system illness, and feather picking is among the reported signs. Look for galvanised wire, cheap clasps and quick-links, zips, costume jewellery, old painted woodwork, curtain and fishing weights, and stained-glass items. Signs beyond the feathers include regurgitation, increased drinking and urination, weakness, and neurological changes.
Both are diagnosable. Radiographs may show metal dense pieces in the digestive tract, and blood levels can be measured, though the sample has to be collected in a way that avoids contamination, which is a reason to use a vet familiar with birds.
Viral feather disease.
Psittacine beak and feather disease and, in budgerigars, polyomavirus produce abnormal, dystrophic, or missing feathers. The critical distinction is that the bird is not doing this to itself, so the head and face are affected too, and beak abnormalities may appear alongside. Testing exists and matters, because these have implications for any other bird in the house.
Airway and airborne irritants.
Smoke, vaping, aerosols, scented candles, plug-in fresheners, and overheated non-stick cookware all irritate skin and airway. They rarely explain a case on their own and they frequently make one worse.
A badly done wing trim.
Bluntly cut primary feathers can prick the flank each time the wing is folded, and the bird responds by attacking the area. It is easy to miss and easy to correct, and it is a good example of why a physical examination beats a theory.
Allergy.
Frequently claimed and poorly documented in parrots. It is not a first-line explanation, and elimination diets improvised at home cause more harm than they solve. Raise it with your vet after the testable causes are excluded.

Head and face feathers are the ones a bird cannot reach with its own beak. Damage there points away from self-plucking and toward disease, or toward a cage mate.
What the workup usually includes
You do not need to request these individually, but knowing the list makes the cost easier to understand and stops the appointment being cut short.
Reading the pattern of damage
The distribution is not a diagnosis, but it is the cheapest information available and it directs the workup.

What never to do
Never fit a collar or a jacket without veterinary direction. Collars stop the bird eating and drinking normally, cause significant distress, and do nothing about the cause. There are cases where a vet fits one to protect broken skin, and that is a clinical decision with monitoring attached.
Never use a bitter, deterrent, or anti-plucking spray. Most are alcohol-based and dry the skin further, some are irritant, and none address a medical cause.
Never hang a mite protector in or near the cage. The traditional ones release an organophosphate vapour, they are toxic to birds in an enclosed space, and the vast majority of indoor parrots do not have mites in the first place.
Never treat for parasites blind. Guessing at mites wastes the window when the actual cause is still treatable, and repeated inappropriate treatment irritates skin that is already inflamed.
Never give leftover antibiotics or steroids from another animal. Systemic steroids in particular have significant adverse effects in birds and are used carefully and for specific reasons.
Never clip the wings as a management strategy for picking. It does not reduce picking, and a poor trim can start it.
Never assume boredom on day one. It is a real contributor, and it is also the explanation that lets a treatable disease run untreated for months.
Never bathe a bird with human shampoo, dish soap, or a medicated product intended for a dog. Plain water misting is the correct home bathing method unless a vet has prescribed something.
Never punish or interrupt the bird sharply when it picks. It increases stress, damages the relationship, and moves the behaviour to when you are not watching.

Regular misting with plain water supports skin that is dry from indoor heating. It is useful supportive care, and it is not a substitute for finding out why the bird is picking.
What helps while the workup is under way
Raise the humidity and bathe the bird regularly with plain water, by misting or by offering a shallow dish or a shower perch. Dry skin is a genuine contributor in heated homes and this costs nothing.
Give the bird a proper dark night. Sleep deprivation is a frequent and easily missed factor, and a bird in a room with a television running until midnight is not getting one.
Take the aerosols, scented products, candles, smoke, and non-stick cookware out of the bird's airspace entirely.
Move the diet toward a formulated base with vegetables if it is not there already, gradually and with veterinary guidance, because abrupt conversions cause birds to stop eating.
Remove any metal item of unknown provenance from the cage now, before the test results come back.
Keep the photograph series going, and keep weighing weekly.
Do not start a large enrichment overhaul at the same time as the medical investigation if you can avoid it. Change several things at once and you will not know which one helped.

When to get help
Go the same day for broken skin, bleeding, a raw or ulcerated area, or a bird that is picking constantly and cannot be settled.
Book this week for any newly damaged or bald area, for damage on the head or face, for abnormal-looking feathers rather than chewed ones, for picking alongside regurgitation, increased drinking, weight loss, or lethargy, and for a cockatiel picking under the wings and thighs.
Bring dated photographs from several angles, the weight record, the full diet including treats and supplements, a list of what the cage, clasps, and toys are made of, the household air, the sleep schedule, when and where the picking started, and what has already been tried.
Ask at the appointment whether skin sampling, faecal testing, blood work, radiographs, and metal levels are being included, and if they are being staged over time, ask what the order is and why. This is a workup where knowing what has and has not been excluded is worth as much as the results themselves.
