Hedgehog wheel and bedding injuries: thread tourniquets and trapped feet

Quick answer
Most hedgehog injuries are caused by ordinary equipment that looked fine on the shelf. The wheel and the fabric are the two things worth auditing first.
Two pieces of standard hedgehog equipment cause most of the serious limb injuries vets see in this species: a wheel with a running surface that a foot can pass through, and fabric bedding that sheds long threads.
Both injure the same way. Something narrow gets around a toe or a leg, the animal keeps moving, and the tissue beyond the constriction dies. A hedgehog will not tell you it has happened. It is a prey species with a strong instinct to conceal weakness, and it hides in a dark house all day, which is why these are usually found late.

A thread wrapped around a toe or a leg is an emergency, and it gets worse on a predictable timetable.
The thread first blocks the veins, which are low pressure and collapse easily. Blood keeps arriving through the arteries but cannot leave, so the part swells. The swelling tightens the thread. Eventually arterial supply fails too and the tissue dies. By the time the toe is black, it is not recoverable.
Do not try to cut an embedded thread out at home. Once swelling has started, the thread is buried in tissue and looks exactly like a skin fold. Owners cut through the toe trying to find it. This needs magnification, good lighting and usually sedation. Keep the hedgehog warm, do not soak the foot, and get a veterinary appointment the same day.

Why hedgehog feet are the weak point
A hedgehog's foot is not built like a rodent's. The toes are long, thin and quite mobile, the animal walks on the whole sole rather than the toes, and there is no broad pad to bridge a gap. When a hedgehog runs, the foot lands and then pushes off, so the toes spread and then trail slightly behind.
That is exactly the movement that finds a hole. On a mesh or barred wheel the toes drop through the gap at the moment of push-off, and the wheel keeps turning. The leg is now anchored to a moving surface and gets levered against the rim. The result can be a fractured toe or leg, a torn joint capsule, or a degloving injury where the skin is stripped from the foot.
The same anatomy is why threads are so dangerous. A single long fibre easily slips into the space between two toes and works its way tight as the foot flexes with every stride on the wheel. A hedgehog that runs for hours in the dark is essentially winching the thread tighter all night.
There is a second reason these injuries are found late. Hedgehogs are active only at night, sleep in a covered house all day, and roll into a ball when handled. Many owners genuinely do not see the underside of the feet for weeks at a time.
Choosing a wheel that cannot injure
The running surface must be solid and continuous.
No mesh, no bars, no gaps between slats, no seam wide enough to take a toe. This single rule prevents most wheel injuries.
The diameter must be large enough that the back stays level.
Watch the hedgehog run and look at the line of the spine from the side. If the back is visibly curved upwards through the whole stride, the wheel is too small for that animal. Judge it on your own hedgehog rather than on a number on the box, because a large adult and a small young one are not the same case.
There must be no shear point at the axle side.
Look at where the wheel meets its stand or support. If there is a narrow moving gap there, a foot, a leg or the loose skin of the skirt can be drawn in. A good design has either a wide clearance or a closed back plate.
The stand must be stable under a running hedgehog.
Hedgehogs are heavy for their size and they run hard. A wheel that rocks, walks across the cage or tips can trap the animal underneath it. Free-standing bucket wheels need a wide base, and cage-mounted wheels need to be bolted, not clipped.
It must come out for cleaning without tools.
This matters more than it sounds. A wheel that is awkward to remove gets cleaned less, which means it also gets inspected less, and inspection is what finds a cracked surface or a rough edge before it opens a wound.

A solid running surface, a stable stand and a fleece hide with no exposed seams. Every item here is one you should be able to take out and inspect in under a minute.
The fabric problem
Fleece liners and fabric hides are popular for good reasons. They are warm, washable, dust free and kinder to feet than loose shavings. They are also the commonest source of the threads that cause constriction injuries.
Terry towelling is the worst offender.
Towelling is made of loops. A toe goes into a loop, the loop pulls, and the toe is caught. Never line an enclosure, a carrier or a sleeping bag with a looped towel.
Fleece is safe only while it is intact.
Fleece does not fray at a raw cut edge the way woven cloth does, which is why it is recommended. What it does do is shed long fibres from worn areas, from seams, and from anywhere a hedgehog has chewed. Old fleece that has been washed many times is not the same material as new fleece.
Sewn hides and tunnels need their seams checked.
The inside seam of a fabric tunnel is where threads appear first, and it is the part you cannot see without turning the item inside out. Turn them inside out every wash.
Knitted or crocheted items are the highest risk of all.
A single pulled loop in a knitted item releases a very long continuous thread. Hand-made cage accessories are common gifts and are worth checking with more suspicion than shop-bought ones.
Replace rather than repair.
Fabric bedding is cheap. A liner that has started to shed fibres will keep shedding, and a hedgehog gets one chance per night to find one.
Staging a constriction injury
Knowing the sequence lets you judge how long the problem has been there, which is genuinely useful information for the vet.

Tissue that reaches the pale or black stages does not recover. What is still avoidable at that point is infection spreading into the foot and up the leg, which is why a black toe still needs a same-day appointment rather than being left to fall off on its own.
What else makes a toe swell
Do not assume every swollen toe is a thread. The distinction changes the treatment.
Pododermatitis.
Sore, thickened or ulcerated skin on the underside of the foot, usually affecting more than one foot, and associated with abrasive or dirty flooring, obesity and a wet enclosure. It develops over weeks rather than overnight.
Fracture or sprain.
Sudden onset, often after a fall or a wheel incident, with pain on handling and swelling around a joint rather than a sharp line partway along a toe.
Mite infestation and skin disease.
Crusting, flaking and quill loss elsewhere on the body as well, and usually itching. Not confined to one digit.
Abscess or bite wound.
Hot, soft, well-defined swelling, sometimes with a scab, often after contact with another animal.
A tumour.
Slow growing, painless at first, usually in an older hedgehog. Hedgehogs have a high burden of neoplastic disease compared with many small pets, so a lump that grows steadily deserves investigation rather than monitoring.
Retained quill sheath or a foreign body.
Localised swelling with a visible entry point.
The feature that most reliably separates a constriction injury from the rest is the sharp line. A thread injury produces normal tissue on one side of a distinct circumferential mark and swollen tissue on the other. Nothing else does that.
Reading the wheel as a health monitor
A hedgehog's wheel is the best behavioural data you will get from a nocturnal animal you never see awake.
Note roughly how much running happens on a normal night. The pattern of droppings on the wheel, the amount of dirt to clean off, and the noise you hear from another room all give you a rough measure without any equipment.
A sudden stop in wheel use has a short list of causes.
Pain from a foot, leg or spinal problem comes first, and is why the foot check should be done before anything else.
Cold is the next. A hedgehog whose enclosure has dropped below its comfortable range becomes sluggish and stops running as it moves towards an attempted hibernation, which is dangerous in this species. Check the enclosure temperature the same day.
Illness of any kind, particularly respiratory infection, will flatten activity.
Neurological disease produces a different picture: the hedgehog tries to run but falls off, wobbles, or cannot hold itself upright on the curve of the wheel. Progressive loss of coordination in a hedgehog needs veterinary assessment rather than a wheel change.
Then the ordinary explanations: the wheel was moved, it is dirty, it has developed a squeak or a wobble that the hedgehog dislikes, or it was replaced with a different one.

While you arrange an appointment, the useful things are warmth, quiet and offered water. Treating the foot itself is not a home job.
The weekly check that finds these early
Do this when the hedgehog is naturally awake, in the evening, in bright light. A hedgehog woken during the day balls up and you will see nothing.
What the vet will want
Bring the equipment, or photographs of it, along with the hedgehog. The cause is often visible in the setup rather than the animal.
Be ready with when the hedgehog was last seen using the wheel normally, when you last checked the feet, what the bedding is made of and how old it is, the wheel type and whether the running surface is solid, the enclosure temperature range, and the current weight compared with previous weights.
Photograph the foot from more than one angle before the appointment, because swelling can change and a hedgehog that has balled up in the carrier may be hard to examine at the clinic without sedation.
Expect the vet to say that a proper examination of a hedgehog's feet often needs sedation or a short general anaesthetic. This is normal for the species and is not a sign that the case is unusually severe. A balled hedgehog cannot be examined properly, and forcing it open causes pain and gets a worse look than sedation does.
What never to do
Do not use an exercise ball. The ventilation slots are sized for rodent feet and a hedgehog's toes drop straight through them, the animal cannot see where it is going, and it cannot get out when it wants to stop.
Do not use a wheel with mesh, bars, or a support bar crossing the running surface, even if the hedgehog appears to manage on it. The injury happens once.
Do not use a saucer-style wheel for a hedgehog. The tilted running surface throws the spine into a permanent curve and offers nothing solid to stop the animal being flung off at speed.
Do not line anything with towelling, and do not use knitted or crocheted accessories without checking them like a hazard.
Do not soak, poultice or apply ointment to a constricted toe. Soaking increases the swelling that is causing the damage, and creams make the tissue slippery and harder for a vet to work with.
Do not remove the wheel permanently after an injury. Hedgehogs need to run, and a hedgehog with no wheel becomes obese quickly, which brings its own set of foot and liver problems. Replace it with a safe one once the vet has cleared the animal.
Do not lubricate a wheel axle with anything not intended for animal use. Any lubricant will end up on the running surface and then on the feet and in the mouth.

Fabric hides are good enrichment when they are hemmed and intact. Check the inside seams every wash and replace them once they start shedding fibres.

When to get help
Contact an exotics or small-mammal veterinarian the same day for any swollen, discoloured, cold or blackened toe, any leg the hedgehog will not use, any visible thread that will not lift free, or any bleeding.
Go the same day for a hedgehog that has been trapped in or under a wheel, even if it seems to be walking, because fractures in a small animal that has rolled into a ball are easy to miss.
Book a routine appointment for a hedgehog that has stopped running with no obvious cause, once you have checked the feet and the enclosure temperature yourself.
While you wait, keep the hedgehog warm and in a quiet dark box with a soft, thread-free lining such as a plain paper towel, offer water, and remove the wheel from the enclosure so the animal cannot repeat the injury before it has been seen.
