Hedgehog heart disease: the breathlessness that gets mistaken for a chest infection
Cardiomyopathy is one of the most important causes of death in pet hedgehogs and one of the most commonly missed, because its first sign is a nocturnal animal quietly doing less on its wheel. Heart muscle disease is one of the most significant causes of death in pet hedgehogs, and it is one of the easiest to miss. The animal is nocturnal, sleeps in a covered bed, balls up when handled, and is hidden under a spiny coat that conceals both body condition and breathing effort.
So the first sign is almost never dramatic. It is a hedgehog that runs less, stops mid-run, sleeps more, and is slower to unroll. By the time breathing is obviously laboured, the heart has been failing for some time.

Do not attempt to unroll it, do not put it in a warm bath, and do not delay to see if it settles. A hedgehog in respiratory distress has very little reserve, and handling or heating a struggling animal can be the thing that tips it over. Put it in a covered carrier with soft bedding, keep it at its usual temperature, and drive.

What is actually happening
The heart is a pump made of muscle. In cardiomyopathy that muscle changes, most often by stretching and thinning, so the chambers enlarge and each contraction becomes weaker.
Two separate problems follow, and they produce different signs.
Less blood goes forward.
Muscles and organs receive less oxygen than they need. In a hedgehog the first tissue to complain is skeletal muscle during exercise, and the only sustained exercise a pet hedgehog does is on its wheel. So reduced wheel distance, and stopping partway through a run, are the earliest practical signs available to an owner.
As it progresses, this shows as general weakness, reluctance to move, slow unrolling, and eventually collapse.
Pressure builds up behind the heart.
Blood that is not being pushed forward efficiently backs up. Which direction it backs up decides what you see.
If the left side is failing, pressure rises in the vessels of the lungs and fluid leaks into the lung tissue. Gas exchange becomes less efficient, so the animal breathes faster and with more effort. This is the breathlessness that gets called a chest infection.
If the right side is failing, pressure rises in the veins returning from the body and fluid accumulates in the abdomen. That gives a swollen belly, which owners often interpret as weight gain, bloat or a mass.
Both can happen in the same animal.
Why the coat hides it
Spines and the muscle layer beneath them mask both the outline of the body and the movement of the chest wall. An owner watching a hedgehog walk across a floor cannot see the breathing effort that would be obvious on a short-coated animal. This is why counting breaths while the animal sleeps is worth more than watching it.
Telling it from the things it looks like
Respiratory infection or pneumonia
The closest look-alike. Both give fast, effortful breathing.
Infection more often comes with discharge from the nose or eyes, sneezing, audible wet or rattling sounds, and an animal that feels warm. Heart failure more often gives quiet breathing that is simply fast and effortful, with no discharge, and often with cold feet and a poor appetite that developed gradually.
These are tendencies, not rules, and the two frequently coexist. The distinction is made properly with imaging, not from the doorway.
Chest tumours
Hedgehogs have a high rate of neoplasia. A mass in the chest, or fluid caused by one, produces exactly the same breathing pattern as heart failure. Weight loss is often more marked and appears earlier.
Hibernation attempt
A pet hedgehog kept too cool may attempt hibernation, which it is not physiologically prepared for. The animal becomes cold to the touch, floppy, uncoordinated and unresponsive. The distinguishing feature is temperature. A hedgehog that is cold and limp needs gradual warming and urgent advice, not cardiac assessment first.
Wobbly hedgehog syndrome
Produces a progressive loss of coordination that starts at the back end, with a wobble and eventually an inability to stand. It affects movement rather than breathing, so an animal that is unsteady but breathing normally points here rather than at the heart.
Obesity
An overweight hedgehog also stops using the wheel and also breathes harder. Weight and heart disease frequently travel together, and obesity makes cardiac signs worse, so the presence of one does not rule out the other.
Anaemia
Produces weakness, exercise intolerance and pale gums. Gum colour is checked in the same examination.
Pain
A hedgehog with dental pain, an injury or a tumour anywhere also does less and eats less. Reduced activity alone is not specific to the heart, which is why the breathing assessment matters.

Gum colour is part of the assessment, and it is far easier to check on a hedgehog that has been handled regularly while well. A distressed, breathless hedgehog should not be wrestled into this position.
The stages, and what each looks like
Early
Wheel distance falls. The hedgehog stops partway through a run and rests. It sleeps a little longer, unrolls a little more slowly, and may be marginally less enthusiastic about food. Weight drifts, in either direction, because fluid can mask loss of muscle. Resting breathing rate is slightly above its usual figure.
Everything here is easy to attribute to age or to the weather, which is why it is usually missed.
Established
Breathing effort becomes visible at rest, with the flanks and abdomen moving. The animal is reluctant to move around the enclosure. Appetite falls more clearly and weight goes down. The abdomen may look or feel fuller. Feet feel cool. The hedgehog may prefer to sit propped rather than curl fully, because curling compresses the chest.
Late
Open-mouth breathing. Gums and the inside of the mouth lose their pink colour and move toward grey or blue. Collapse, sometimes triggered by handling or by a short burst of activity. Sudden death is possible at any point once the disease is established.
The one measurement worth learning
Count the resting breathing rate.
This is the standard way owners of cardiac dogs and cats monitor at home, and it works for a hedgehog too, with one adjustment: you compare against your own animal rather than against a published figure.
How to do it
Wait until the hedgehog is asleep, settled and undisturbed, in its usual bed at its usual temperature. Watch the rise and fall of the flank, count the breaths over a fixed period using a timer, and write the number down with the date.
Do it while the hedgehog is healthy.
That is the whole point. A single number in a sick animal tells you little. A number that has climbed above that animal's own established resting figure, and that keeps climbing, is meaningful information that a vet can act on.
Do not count while the animal is awake, warm from activity, or being handled.
All three raise the rate and make the measurement useless.
Weigh weekly too.
In heart disease, weight is complicated by fluid, so a stable weight does not mean a stable animal. But a record still helps, particularly for spotting the fluid accumulation that follows right-sided failure.
What raises the risk
Age.
Cardiomyopathy in pet hedgehogs is largely a disease of middle-aged and older animals, which in this species arrives sooner than owners expect.
Obesity.
Extra mass increases the work the heart has to do, and an overweight hedgehog that has already stopped exercising is in a loop that reinforces itself.
Inactivity and a small enclosure.
Not a cause of the muscle disease, but it removes the wheel-distance signal that would otherwise have told you early. A hedgehog with no wheel gives you far less information.
Genetics.
The captive population of African pygmy hedgehogs descends from a limited founder group, and inherited predisposition is widely suspected. This is a reason to ask a breeder about the health and longevity of related animals, not a reason to assume any individual is doomed.
Diet.
Dietary factors have been proposed, including specific amino acids, but this is not established in hedgehogs. Do not add supplements on that theory. Feed a diet appropriate for an insectivore and take supplement decisions with your vet.

The wheel is a monitoring instrument as well as enrichment. A hedgehog that used to run all night and now barely turns it has told you something before any other sign appears.
What happens at the vet, and why sedation may be suggested
Listening to a hedgehog's chest is genuinely difficult. A hedgehog that balls up presses spines and muscle over the chest wall, and a stressed animal huffs and clicks loudly enough to drown out heart sounds.
That is why a proper cardiac assessment in a hedgehog usually requires sedation or a light anaesthetic. This is not the practice being over-cautious. It is the only way to get diagnostic images and a usable examination.
Expect some combination of chest radiographs to look at heart size and lung fluid, ultrasound of the heart if the practice has the equipment and experience, and blood tests to look at other organs.
Treatment is management.
Medication can reduce the fluid load, support the failing muscle and improve quality of life, often considerably. It does not repair the muscle. Started early, it buys good months. Started when the animal is already in crisis, it buys much less.

A carrier that is already lined and reachable matters more than anything in a first aid kit, because a hedgehog in cardiac crisis needs a quiet journey rather than treatment at home.
Living with a diagnosis
What never to do
Do not treat reduced wheel use as laziness or as the animal getting older. Hedgehogs that can run, run.
Do not warm a breathless hedgehog in a bath or on a hot surface. Heat increases the demand on a failing heart, and a bath in a struggling animal risks aspiration.
Do not force a distressed hedgehog to unroll for examination. Take it to the vet in a carrier and let them sedate it if necessary.
Do not give human heart medication, diuretics or supplements. Doses do not scale down safely, and diuretics given without a diagnosis can be dangerous.
Do not withhold food from a hedgehog that is eating less. Reduced intake in a cardiac animal is a sign to report, not something to correct by fasting or by forcing.
Do not assume the diagnosis is a chest infection because a course of antibiotics helped a little. Some improvement can occur for other reasons, and a hedgehog that relapses after treatment needs imaging rather than a second course.
Do not wait for the weekend to pass. Cardiac decompensation in a hedgehog moves in hours.
