Fish get fat, and the fat does not stay where you can see it. It accumulates around the internal organs and inside the liver, and it is the liver that gives out first.
The reason this matters more in fish than in most pets is anatomy. A fish body cavity is small and full, and fat deposited inside it presses on the gut, the gonads and the swim bladder. That is why an overfed fancy goldfish so often ends up floating or sinking, and why the buoyancy problem looks like a mechanical fault when it is really a feeding one.

Why fat matters more inside a fish
Fish deposit surplus energy in three places: under the skin, in the mesentery around the gut, and in the liver itself.
The liver deposits are the dangerous ones. Liver cells fill with fat droplets, the organ enlarges and becomes pale, and its capacity to process nutrients, clear waste and produce proteins declines. This is the same process that causes liver failure in overfed birds and in cats that stop eating, and it works the same way in fish. Once the liver is significantly affected, a fish that appeared perfectly healthy can deteriorate quickly under any additional stress, which is why owners so often describe the death as sudden.
The mesenteric fat causes mechanical problems. A fish body cavity has no spare room, and fat around the gut presses on the swim bladder, the intestine and the reproductive organs. Compressed gut means slower transit and constipation. Compressed swim bladder means the fish cannot fine-tune its position in the water.
Fancy goldfish are the worst affected because selective breeding has already shortened and rounded the body, folding the intestine into a small space and distorting the swim bladder. Adding internal fat to that anatomy is what produces the classic overweight fancy goldfish that floats nose down, or drifts up after every meal.
Reproduction suffers too. Heavily fat fish frequently fail to spawn, and in some species the fat replaces functional gonad tissue.
What good condition looks like, and how to check it
Look from above.
This is the view that shows fat and it is the view most owners never use. A well conditioned fish is widest just behind the gill covers and tapers steadily from there. An overweight fish looks broad through the whole front half and loses the taper.
Look from the side.
The belly line should curve gently. A pronounced downward bulge, especially one that begins immediately behind the pectoral fins, indicates fat rather than a full stomach.
Watch the fish move.
A fit fish accelerates instantly, turns sharply, and holds position in a current. A heavy fish is slower to start, slower to turn, and drifts.
Know your species' normal shape.
This matters enormously. A healthy oranda or ranchu is a deep, round fish. A healthy zebra danio is a torpedo. Judging one against the other produces nonsense. Compare your fish against good photographs of a healthy adult of the same variety, not against a generic idea of a fish.
Distinguish full from fat.
A fish that has just eaten well has a temporarily fuller belly that returns to normal within hours. Check condition before feeding, at the same time of day.
Distinguish female from fat.
Females of many species become noticeably deeper when carrying eggs, and in livebearers a gravid female is obvious. That change is low in the body, symmetrical, and cyclical.
What actually causes it
The number of feeding events, not the size of them.
Most overfeeding is accumulated. A pinch in the morning, a pinch when someone walks past, a pinch from a child, a treat in the evening. Fish beg convincingly and they do not stop when they are full, because in the wild there is no reason for that instinct to exist.
Feeding to a behaviour rather than to a portion.
The habit of feeding until the fish lose interest guarantees overfeeding, because most fish will keep eating well past sufficiency.
Rich foods used as staples.
Bloodworm, beef heart, brine shrimp, and feeder fish are all energy dense and are frequently fed daily because fish eat them enthusiastically. Bloodworm in particular is a treat in most balanced diets and a staple in far too many tanks.
Single-food diets in omnivores.
A goldfish or a barb fed exclusively on a high-protein pellet has no vegetable component and gets more energy per mouthful than the species is built for.
Stale food.
An opened tub of flake or pellet sits in a warm room for months, and the fats in it oxidise. Oxidised fats damage the liver and destroy the vitamin E that would otherwise protect it. Buy small containers, keep them cool and sealed, and throw away what you have not used within a few months.
Not enough room to swim.
A fish in a tank too short to swim in cannot burn what it eats. Tank length matters more than volume for active species, and pond and tank flow gives fish something to work against.
Winter feeding of pond fish.
Coldwater fish slow their entire metabolism as the water cools, and below a certain temperature the digestive system effectively stops. Food eaten then sits undigested. This is a serious and common autumn mistake.

Weighing or counting the daily portion, once, replaces a dozen daily judgement calls and stops the total creeping upward.
Correcting it safely
Go slowly.
This is the most important rule and the one most often broken. A fish with a fatty liver that is suddenly fasted mobilises fat into a liver that is already struggling, which makes it worse rather than better. Reduce the daily total in steps over weeks, not overnight.
Count and measure, once.
Decide the daily amount, measure it out, and put it in a small container each morning. Everything the fish gets that day comes from that container. This single change fixes most household overfeeding, because it makes multiple feeders visible.
Feed smaller portions more often rather than one large meal.
Fish guts are short and work best with small frequent inputs. Splitting the same daily amount into more feedings improves digestion and reduces the amount of undigested waste going into the water.
Change the food, not just the amount.
Move rich treats to once or twice a week. For omnivores, add appropriate vegetable matter such as blanched greens or a vegetable-based pellet. For herbivores, make that the base.
Make them work for it.
Scatter food rather than dropping it in one place. Use a feeding ring only where you need to, since it concentrates food and reduces movement. Sinking food for bottom dwellers spreads foraging across the tank.
Increase swimming.
More length, better flow, and fewer obstacles in the open water. For pond fish, this happens naturally in warmer months.
Use fast days appropriately.
A weekly day without food is reasonable for healthy adult fish of many species and gives the gut time to clear. It is not appropriate for fry, for very small species with high metabolic rates, for a fish that is already unwell, or as the whole weight loss plan.
Measure progress properly.
Photograph the fish from directly above and from the side, in the same spot with the same lighting, once a fortnight. Change over weeks is invisible day to day and obvious in a photograph series.
When it has gone beyond diet
Some signs mean the problem is no longer something feeding changes will fix on their own.
Appetite loss in a fat fish.
Concerning rather than reassuring. It suggests the liver is failing rather than that the fish has finally had enough.
Persistent buoyancy problems.
Floating, sinking, or an inability to hold position that continues after feeding has been corrected. Some of this is reversible in the early stages and some is not, particularly in heavily modified fancy goldfish.
Pale gills.
Gills should be a rich red. Pale or washed-out gills suggest anaemia or poor circulation and warrant advice.
Rapid abdominal swelling.
Especially with raised scales. That is fluid, not fat, and it is a different and more serious problem.
Sudden death of an apparently healthy fat fish.
Unfortunately common with advanced fatty liver, and a reason to act on the shape of a fish before anything else changes.
Vets who work with fish can assess a case properly, and in valuable individuals such as koi they will examine, sample and image the fish. That is worth knowing about before a fish is at the point of collapse.
What never to do
Do not starve a fat fish to correct it quickly.
Do not feed bloodworm, beef heart or other rich foods as a daily staple.
Do not feed pond fish in cold water.
Do not use a food tub that has been open for many months, however much is left in it.
Do not judge a fancy goldfish by the outline of a common goldfish, or any fish against a species with a different body plan.
Do not treat a swollen belly with fasting when the scales are raised. That is fluid, and it needs urgent advice.
Do not let visitors and children feed the tank without a portion system. This is the most common single reason a carefully managed tank becomes an overfed one.
Do not assume a fish that eats eagerly is hungry. Fish are opportunistic feeders and enthusiasm carries no information about need.

The target is a fish that eats a measured portion, clears it quickly, and goes back to swimming rather than searching.
Bring photographs from above and from the side taken over time, the species and variety, the exact food including how long the tub has been open, the daily amount and how many people feed, the tank or pond dimensions, the temperature, and your water test results. Feeding history is the diagnosis in most of these cases, and the detail that turns out to matter is usually the one an owner assumed was too small to mention.
