Fish tuberculosis: the wasting disease with no cure, and the risk to your hands

It matters for two reasons. There is no reliable cure once fish are affected, so the response is biosecurity rather than medication. And the bacteria involved can infect people through broken skin, which is why gloves and covered cuts are part of ordinary aquarium keeping.
- Suspect it when fish waste away over weeks while eating, and the water parameters are normal.
- It cannot be diagnosed by looking. Confirmation needs laboratory examination of tissue, usually from a fish that has died.

Why this disease looks the way it does
Slow bacteria produce slow disease.
Mycobacteria replicate slowly compared with the bacteria behind fin rot or a septicaemia. The infection therefore establishes gradually, the immune system walls parts of it off into nodules, and the fish declines over weeks or months rather than crashing over days.
That is why the classic history is a tank that loses one fish every few weeks, and why owners often describe the tank as fine apart from a few fish that never do well.
The wasting is internal.
Granulomas in the liver, kidney, spleen and gut interfere with digestion, absorption and metabolism. The fish continues to feed but cannot convert food into condition. That combination, eating well and getting thinner, is unusual and it is the most useful clue you have.
The spine curves because the disease reaches the tissue around it.
Progressive infection near the vertebral column, together with the general wasting, produces the bent or hunched outline seen in advanced cases. It is a late sign, not an early one.
Stress is the trigger, not the cause.
The organisms are common in aquatic environments. What separates a tank that never sees clinical disease from one that loses fish is usually crowding, poor water quality, unstable temperature, aggression, and repeated introductions of new fish.
What you actually see, stage by stage

A well fish has a full outline behind the head, smooth intact skin, clear eyes and even colour. Compare any fish you are worried about with the healthiest one in the same tank rather than with a memory.
Early.
Slight loss of condition, best seen as a narrowing of the back and belly when the fish is viewed from above or head on. Colour looks slightly flat. The fish still eats and still shoals.
Established.
Obvious thinness, a hollow belly, a visible dorsal ridge. Reduced activity, hanging away from the group, sometimes hiding. Fins may look ragged. Scales can begin to protrude in patches.
Late.
A curved spine, ulcers that do not heal, one or both eyes bulging, loss of scales, pale patches, and a fish that is clearly failing. Some fish die without ever reaching this stage.
Across the tank.
Losses spread out over time. Fish that arrived healthy start declining months later. Breeding stock produce weak fry. New fish added to the tank do poorly.
What else looks like this
Wasting in fish has a long differential list, and most of the alternatives are more treatable, which is exactly why a proper look matters.
Internal parasites.
Weight loss with a good appetite is also the classic picture with intestinal worms and some protozoa. Pale, stringy or white faeces point this way, and specific treatment often works.
Underfeeding or competition.
In a busy tank, timid or slower fish simply do not get enough. The tell is that the affected fish are the ones lowest in the pecking order, and the problem improves with targeted feeding.
Poor diet.
A monotonous or unsuitable diet produces gradual decline in condition across the tank rather than in individuals.
Water quality.
Chronic exposure to nitrogenous waste damages gills and reduces growth. Test rather than assume, because a tank can look clean and still be wrong.
Old age.
Small tropical species have short lives, and an aged fish thins and slows. Age-related decline affects the oldest fish, not new arrivals.
Tumours and organ disease.
Common in older fish, and usually affect an individual rather than spreading through the tank.
Skeletal deformity from other causes.
A bent spine can result from developmental problems, injury, nutritional deficiency or, occasionally, electrical injury. A curve present from a young age in a fish that grows normally is different from a curve that develops in an adult that is wasting.
The pattern that points toward mycobacteriosis specifically is several fish, over months, wasting while eating, in a tank whose water parameters are acceptable, sometimes with non-healing ulcers.
Confirming it
You cannot diagnose this from the outside, and anyone who tells you otherwise is guessing.
The reliable test is on tissue.
A vet with aquatic experience will usually suggest submitting a freshly dead or humanely euthanised affected fish for post-mortem examination. Granulomas are looked for in the organs, and special stains identify acid-fast organisms.
That feels drastic and it is often the kindest option.
Knowing what is in the tank changes everything: whether to keep treating, whether to add fish, whether to break the tank down, and how careful to be with your own hands. One fish provides that answer for the whole system.
Keep a sample properly.
If a fish dies and you want it examined, ask the clinic first. Refrigerate, do not freeze, and get it there quickly. A frozen fish is much harder to interpret.
Log what you see.
Dates, which fish, what they looked like, water test results, and any changes to the tank. In a slow disease the timeline is the diagnosis.

A separate container, its own net and siphon, and a written log. Equipment that moves between tanks is one of the main ways this disease spreads through a fishroom.
Managing a tank that has it
Stop moving things.
No fish out, no fish in, no plants, decor, filter media, nets, siphons or buckets shared with another tank. Equipment is the commonest route between systems in a house with more than one aquarium.
Reduce the pressure on the survivors.
Lower stocking density, keep temperature stable, keep nitrogenous waste low with regular partial water changes, feed a good varied diet, and remove aggression by rehoming the aggressor within the same system rather than out of it.
Remove affected fish humanely and promptly.
A visibly wasting or ulcerated fish is both suffering and shedding organisms. Leaving it in the tank prolongs both. Humane euthanasia methods for fish should be discussed with your vet in advance so you are not deciding under pressure.
Do not restock into the problem.
Adding new fish to a tank with ongoing losses is the single most common way owners extend the outbreak by a year.
Consider the endpoint honestly.
In a badly affected system, the eventual answer is often to let the population run down, then break the tank down and disinfect everything thoroughly before starting again. That is a hard decision and it is sometimes the right one.
Ask about medication realistically.
There is no dependable cure available to hobbyists, and the aggressive treatment regimes described online are largely unproven, hard on the fish, and damaging to the filter. Any treatment decision belongs with a vet who has seen the case.

Water changes, stable conditions and dechlorinated top-ups still matter enormously. What does not work is escalating through medication bottles hoping one of them reaches a slow-growing organism inside a granuloma.
Protecting yourself
This is the part most fishkeeping advice leaves out, and it is straightforward.
The route in is broken skin.
The organisms enter through cuts, grazes, cracked skin around the nails, and scratches from decor or from a fish spine. Intact skin is a good barrier.
What it looks like in people.
Typically a slow-growing bump or sore on a hand, finger or forearm, appearing weeks after the exposure, that does not respond to ordinary treatment and can spread as a line of nodules up the arm. It is often mistaken for something else for months.
The single most useful thing you can do.
Tell your doctor that you keep an aquarium, unprompted, when you show them a skin lesion on your hand or arm. That one sentence redirects the investigation and shortens the diagnosis considerably.
Who needs to be more careful.
Anyone with reduced immunity, whether from illness or from medication, along with anyone with eczema or broken skin on their hands. If that describes you, gloves are not optional.
Prevention that actually works
Quarantine every new fish.
A separate tank, its own equipment, and a period long enough for problems to declare themselves. Most tank-wide disease arrives with a fish that looked fine in the shop.
Buy from suppliers who look after their systems.
Look at the whole shop rather than the individual fish: are there dead fish in the tanks, are the systems shared, do the fish look well conditioned.
Do not overstock.
Crowding is the reliable amplifier of every infectious disease in an aquarium.
Keep conditions stable.
Temperature swings, unstable pH, accumulating nitrate and irregular maintenance all reduce a fish's ability to contain an infection it may already carry.
Feed properly.
Varied, appropriate, and not so much that waste accumulates.
Do not move fish casually between tanks.
Every transfer is an opportunity.
What never to do
Do not add new fish to a tank that is losing fish.
Do not share nets, siphons, buckets, plants or decor between tanks.
Do not put bare hands with cuts into an aquarium.
Do not siphon by mouth, ever.
Do not treat repeatedly with escalating medications hoping to hit it.
Do not release aquarium fish or pour aquarium water into a garden pond, a stream or any natural water.
Do not wash tank equipment where food is prepared.
Do not assume that because the tank looks clean, the disease cannot be present.
