Ich and velvet in aquarium fish: catching an outbreak early
Fish have no external ears, so there is no ear infection to catch. The real early-detection target is a skin parasite outbreak. This guide shows how to tell ich from velvet, what rubbing means before spots appear, and why treatment must run its full course.

Why there is no ear to check
A fish detects sound and movement through an inner ear sealed inside the skull, and through the lateral line, a row of sensory pores running along each flank.
Neither structure opens to the outside in a way that collects wax, moisture or debris. There is nothing to swab and nothing to look into.
What is exposed is the entire body surface. Skin, fins and gills sit in direct contact with the water at all times, with only the slime coat between the fish and everything living in the tank.
That is why external parasites are the real early-detection target. They live on the surface the fish cannot get away from.
What the body surface should look like
Watch the fish through the glass in normal light, then again with a small torch angled from the side. The angled beam is what makes fine dust visible.
Healthy scales lie flat and reflect light evenly across the flank. Fins are held open and clear, without cloudiness at the edges.
The slime coat gives a smooth, slightly glossy look. It should not appear patchy, stringy, or grey.
There should be no flicking, no dashing sideways against gravel or decor, and no fish parked in a corner with fins held tight to the body.

Write down what you see each day. A three-day trend tells you far more than a single evening's observation.
Changes that mean act today
Flicking and glancing against surfaces.
This is the earliest sign, and it appears before any spot is visible. It also has non-parasitic causes, which is why the water test comes first.
Salt-grain spots.
Ich cysts are discrete, roughly uniform in size, and sit on fins as well as the body. They do not smear or move.
Gold or rust dust.
Velvet is finer than ich and easy to miss under front lighting. It concentrates on the gills and head early, which is why affected fish often breathe hard before they look dusty.
Clamped fins with fast breathing.
Fins held tight against the body plus rapid gill movement means the gills are already involved. Treat this as urgent regardless of what you can see on the skin.

Even, flat scales and clear fin edges are the baseline you are comparing against.
Why treatment has to run the full course
Both parasites spend part of their life cycle attached to the fish, protected under a layer of skin, and part of it free in the water.
Medication reaches the free-swimming stage. It does not reach the encysted stage on the fish.
That is the mechanism behind the most common failure. The spots disappear, the keeper stops treating, and the next generation hatches out of the substrate a few days later.
Complete the full course specified on the product you are using, and keep going for the stated period after the last visible spot has gone.
Warmer water speeds the cycle up, which shortens treatment but also shortens the time you have before the infestation multiplies. Any temperature change must stay inside the range your species tolerates and be made slowly.
The routine that catches it early
What never to do
Do not reach for an antibiotic. Ich and velvet are protozoan parasites, and antibacterial treatment adds chemical load without touching them.
Do not treat only the fish that shows spots. The parasite reproduces in the tank, so treating one animal in isolation leaves the reservoir intact.
Do not leave carbon or chemical media running in the filter during treatment. It strips the medication out of the water.
Do not stop the course early because the fish looks clean.

When to get help
Contact an aquatic or exotics veterinarian the same day if several fish are affected at once, if breathing is laboured, or if fish are dying during treatment.
Get advice before treating if your tank holds invertebrates, scaleless species such as loaches and many catfish, or fish that are already weak. Several standard treatments are poorly tolerated by these animals.

A recovered fish holds its fins open, swims through the whole tank, and stops flicking at decor.
Bring your water test numbers, the tank volume, the stocking list, the date and source of anything recently added, and photographs of the affected fish taken through the glass.
That history usually names the source of the outbreak faster than examining the fish does.
