Why the gills make this so fast
Gills are built to extract dissolved oxygen efficiently. That means a huge surface area, an extremely thin barrier, and blood flowing directly against the water.
Anything dissolved in the water crosses that barrier as readily as oxygen does. There is no equivalent of skin thickness or of a digestive system to slow absorption down.
Fish also cannot leave. A land animal moves away from a smell. A fish continues to pass the contaminated water over its gills at the same rate.
Add an air pump and the situation worsens. The pump sits in the room, draws in whatever is in the air, and bubbles it through the tank.
This combination is why aquarium losses from household chemicals are so often total rather than partial.
Which animals are affected first
Invertebrates.
Shrimp, snails and other invertebrates are the most sensitive occupants of most tanks. Insecticides act on invertebrate nervous systems by design, and copper is toxic to them at low concentrations.
Dead shrimp with the fish still swimming is a strong early signal of chemical exposure.
Scaleless and thin-scaled fish.
Loaches, many catfish, eels and similar species absorb dissolved substances more readily. Many treatments carry a specific warning for them.
Small fish and fry.
A smaller body means a higher exposure per unit of body mass, so small and young fish show signs first.
Fish already under strain.
A fish recovering from illness, poor water quality, or a recent move has less margin than a healthy one.
This variation is real and species-linked, but it is not something that can be tested for in an individual fish. Treat it as a property of the species you keep, and read every product label against your stocking list.
Changes that mean act immediately
Darting, spiralling, or crashing into decor.
Erratic swimming that appears suddenly across several fish points to a neurological effect. This is an emergency.
Sudden surface gasping across the whole tank.
Chemical gill damage reduces oxygen uptake immediately, so it looks like an oxygen crisis.
Excess mucus, a cloudy film on the body, or clamped fins.
A heavy slime response is the skin's reaction to an irritant in the water.
Invertebrate deaths.
Any unexplained shrimp or snail loss deserves investigation as a chemical event.
The routine that prevents exposure
What never to do
Do not add another chemical to counteract the first. Mixing products is a common way to turn a survivable exposure into a fatal one.
Do not treat an entire display tank with a copper product without first confirming that every occupant tolerates copper.
Do not use flea, tick or worming products intended for other pets anywhere near the tank, or on hands that will enter it.
Do not run carbon and medication at the same time under normal circumstances, because carbon removes the medication. During a poisoning event, removing everything is exactly what you want.
Do not do the water change with untreated tap water. Chlorine adds a second insult to already damaged gills.

When to get help
Contact an aquatic or exotics veterinarian immediately if fish are swimming erratically, if several fish die at once, or if you know a chemical entered the room.
Do not wait to see whether it settles. Chemical exposure progresses over minutes and hours, and the water change you do in the first hour matters more than anything done later.
Bring the product name and its ingredient list, the tank volume, the stocking list including invertebrates, the time of exposure, and what you have already done.
Keep the packaging. The active ingredient determines whether the exposure is likely to be survivable and what the follow-up should be.
