Quick answer
Coat and vulva are the two things worth photographing regularly in a ferret. Both change before the animal looks ill.
A female ferret that comes into season and is not mated does not come out of it. Ferrets ovulate only in response to mating, so without that trigger the ovarian follicles persist and oestrogen stays high for months. Sustained high oestrogen shuts down the bone marrow, and a jill left in that state develops anaemia that is frequently fatal by the time an owner notices anything is wrong.
That makes the decision about desexing a medical one rather than a lifestyle one. It is complicated by the fact that surgical neutering in ferrets is linked to adrenal disease years later, so the real question is not whether to intervene but which method of interrupting the cycle suits this animal. That conversation belongs with a ferret vet before the first season, not during it.
- Ferrets are long-day breeders. Increasing daylight, including artificial household light, starts the season.

Prolonged oestrus in an unmated jill is a slow emergency.
Oestrogen suppresses the bone marrow, which produces red cells, white cells and platelets. The animal therefore develops three problems at once: anaemia, vulnerability to infection, and a tendency to bleed. Because red cells are replaced slowly, weeks pass before the jill looks unwell, and by then the marrow damage may not be reversible even after the season is ended.
Pale gums, pale inner ears, weakness, black tarry stool or unexplained bruising in an intact jill are an emergency, not a wait-and-see. Some cases need a transfusion, and not every clinic can transfuse a ferret. It is worth knowing which local practice can before you need one.

Why the ferret reproductive cycle behaves this way
Ferrets take their reproductive cue from day length. As daylight lengthens, jills come into season and hobs come into rut, and both states persist while the days stay long. Indoors, with lamps on into the evening for most of the year, that signal is blurred and often extended.
The second feature is induced ovulation. In most mammals, ovulation happens on a schedule and the cycle completes whether or not mating occurs. In ferrets, ovulation is triggered by mating itself. If mating does not happen, the follicles do not release, the cycle does not close, and oestrogen production continues.
For a wild polecat that is an efficient arrangement. Eggs are released only when they can be fertilised, and a solitary hunter that meets a mate infrequently wastes nothing. For a single pet jill in a household with no hob, the same mechanism produces months of unopposed oestrogen.
What oestrogen does to the marrow.
Bone marrow contains the stem cells that produce red cells, white cells and platelets. Sustained high oestrogen suppresses them. The clinical picture that follows has three strands: anaemia as red cells are not replaced, infection risk as white cells fall, and bleeding as platelets fall.
The reason this is so often caught late is that red cells have a long lifespan. Production can already have stopped while the existing cells are still circulating, so the ferret looks and behaves normally for weeks after the damage begins. By the time the gums are pale, the marrow has been suppressed for some time, and stopping the season at that point does not guarantee it recovers.
What a normal season looks like, and what a failing one looks like
A season starting normally.
The vulva swells over a few days, becoming obviously larger and firmer. The jill may be restless, more vocal, and noticeably stronger smelling. She is otherwise well, eating and playing normally.
Weeks in, without intervention.
The vulva stays swollen. Hair begins to thin symmetrically, typically over the tail, rump and flanks, because oestrogen affects the hair cycle. The jill is a little quieter. This is the stage at which the problem is still very reversible and is most often dismissed.
Established marrow suppression.
Gums, the inside of the ear and the skin of the nose lose colour. Energy drops sharply and the ferret sleeps more even by ferret standards. Appetite falls. She may feel thin over the spine.
Late.
Bruising or pinpoint bleeding under the skin, bleeding from the gums, black tarry stool from bleeding into the gut, laboured breathing on minimal exertion, and collapse. Secondary infections appear because the white cells have gone too. This stage carries a poor outlook and needs intensive care.
The look-alikes

A monthly side-on photograph is what makes hair loss obvious. Both oestrogen-driven and adrenal hair loss begin at the tail base and move forward.

The options for an intact jill
The correct choice depends on the individual, on what is available where you live, and on whether you ever intend to breed. All of them are veterinary decisions. Only the last item on this list is not an option.
Surgical neutering.
Permanent and completely removes the oestrus risk. The cost is that removing the gonads also removes the feedback that quietens the pituitary, which is the mechanism implicated in ferret adrenal disease years later. The age at which it is done appears to matter, which is one reason a conversation before the first season is worth having.
A hormonal implant.
An implant that suppresses the reproductive axis will take a jill out of season and keep her out of it, and because it acts higher up the chain it does not create the same unopposed pituitary signal that surgery does. It is reversible and it needs repeating. Availability and licensing vary by country.
A hormone injection to end the season.
Commonly known as the jill jab, given to bring a jill out of oestrus. It works, it needs repeating each season, and it carries its own risks including uterine infection. Your vet will weigh that against the alternatives.
Mating with a vasectomised hob.
Mating triggers ovulation, so a vasectomised hob ends the season without producing kits. It requires access to such a hob, and it brings the usual considerations of mating injury, stress and disease transmission between animals.
Breeding her.
A legitimate choice and a serious undertaking, not a treatment for a medical problem. It commits you to a litter, to homes for them, and to the risks of pregnancy and lactation in a small carnivore.
Waiting to see whether the season ends.
Not an option. It is the thing that kills jills.
The hob side of the same coin
Intact hobs in rut develop a strong musky odour, a coat stained yellow by skin secretions, urine marking, and often aggression toward other ferrets. None of that is illness, but it is difficult to live with and it is a common reason hobs are surrendered.
Castration removes it and carries the same adrenal association as spaying. A vasectomy leaves the hormones and the behaviour intact while preventing kits, which is exactly what you want if the hob's job is to bring jills out of season, and exactly what you do not want if the problem is the smell.
The important hob emergency is urinary. Adrenal disease in a castrated hob commonly enlarges the prostate, which surrounds the urethra. A hob straining in the litter tray and producing only drops, or nothing, is obstructed and that is a same-day emergency in which the bladder can rupture.
Adrenal disease: why neutering is not a free decision
Removing the gonads takes away the sex hormones but not the pituitary hormones that were calling for them. With no gonadal feedback, that signal continues unopposed. Adrenal cortical tissue carries receptors for those same signals, and under continuous stimulation it can enlarge and start producing sex steroids itself.
The result is a ferret that behaves hormonally without gonads: symmetrical hair loss beginning at the tail base, itching, a swollen vulva in a spayed jill, a return of aggression, and prostatic enlargement in a castrated hob.
Light is the part owners can act on.
Because the whole axis is driven by day length, a ferret living under household lighting from morning until late evening gets a signal of endless summer. Providing a genuinely dark, uninterrupted night, and letting the room follow the actual seasons where possible, is one of the few management levers an owner holds directly.
What the vet will ask for and do
Expect a blood count as the test that decides urgency. Packed cell volume is the number that will drive the plan. Expect ultrasound to look at the adrenal glands and to search for an ovarian remnant in a supposedly spayed jill, and expect a frank conversation about transfusion availability if the anaemia is severe.
What never to do
Never wait out a season to see whether it ends by itself.
Never assume a ferret sold as a pet is neutered. Practice varies enormously between countries and between sources, and a rescue with no history should be assumed intact until proven otherwise.
Never dismiss a swollen vulva in a spayed jill. That finding has two explanations and neither is benign.
Never read pale gums as tiredness. Check gum colour in daylight against how the same ferret looked last month.
Never give human iron tablets, tonics or supplements to an anaemic ferret. They do not address marrow suppression and iron overdose is toxic.
Never house an intact hob and an intact jill together without a decision already made about what happens if she conceives.
The routine that catches it early

Monitoring is part of the setup, not an extra task: a scale by the cage and a note of the date turns a vague impression into evidence.

The aim is an animal whose season is managed on a plan you chose, rather than one you discover.
Go immediately for pale gums, pale ears or nose, weakness, bruising, bleeding from the gums, black tarry stool, or laboured breathing in an intact jill. Say on the phone that she is an unspayed female who has been in season, because it changes what the clinic prepares.
Go immediately for any hob straining to urinate or producing only drops.
Book within days for a vulva that has been swollen for weeks in an intact jill, or for a swollen vulva in a jill you believe was spayed.
