Cat: indoor-only versus a secure outdoor enclosure
The real choice is contained versus uncontained. Containment removes the causes of sudden death in cats and replaces them with weight gain, cystitis and boredom, all of which are manageable. A cat in a well built enclosure and a cat living entirely indoors are both contained, and both avoid the causes of death that free roaming adds: road traffic, fight wounds, poisoning, being shut into a shed or garage, and theft.
The trade is that containment moves every risk indoors. Contained cats gain weight more easily, and inactivity plus stress feed straight into the urinary problems that put cats in emergency clinics. An enclosure solves part of that. It does not solve all of it.

Why the free roaming risks are the ones that kill
Owners weigh this decision on quality of life. The mortality data behind it is much simpler than that: the things that kill free-roaming cats are sudden and they are all outside.
Road traffic.
Cats are crepuscular. They hunt at dawn and dusk, which is exactly when drivers see least. A cat crossing a road does not treat a car as a predator to be avoided; it treats it as a large moving object and often freezes.
Bite wounds from other cats.
Cat canines are thin and deep. They inject bacteria under the skin and the puncture seals within hours, which is why an abscess appears three to seven days later as a hot painful swelling rather than at the time of the fight. Those same bites are the main route for feline immunodeficiency virus, and a significant route for feline leukaemia virus.
Poisoning.
Antifreeze is the classic one because it tastes sweet and a small volume causes irreversible kidney failure. Rodenticides reach cats twice over: directly, and through eating a poisoned rodent. Lily pollen brought in on a coat is enough to cause acute kidney injury in a cat that then grooms it off.
Confinement in a building.
Sheds, garages, greenhouses, removal vans and neighbours' outbuildings account for a large share of cats reported missing. A cat shut in a shed on a Friday is in trouble by Monday.
Predation and dogs.
Regional, but real. In areas with coyotes, foxes, birds of prey or loose dogs, small cats and kittens are prey.
None of these are managed by a responsible owner being careful. They are managed by the cat not being there.
What indoor-only actually costs
The opposite framing is equally dishonest. Indoor-only is not a risk-free default; it just trades sudden risks for slow ones.
Weight gain.
An indoor cat's energy expenditure drops substantially, and most owners do not reduce the food. Neutering compounds it. Obesity in cats is not cosmetic: it drives diabetes mellitus, worsens arthritis and makes a cat unable to groom its own lower back.
Feline idiopathic cystitis.
This is the single most important indoor-cat disease. It is a sterile, stress-associated bladder inflammation, and its triggers are the things that go wrong indoors: conflict with another cat, a litter tray in a busy spot, an unpredictable routine, boredom, and low water intake. In male cats it can progress to a urethral obstruction, which is a same-day emergency.
Inter-cat tension.
Outdoors, cats resolve social pressure by using space and time separately. Indoors they cannot. Two cats that "get along" in a garden can be in constant low-grade conflict in a flat, which shows up as blocking, staring, litter tray avoidance and overgrooming rather than as fighting.
Boredom-linked behaviour.
Overgrooming, night activity, furniture destruction and attention-seeking aggression all rise when a cat has nothing to do. These have their own guides in this library; the point here is that choosing indoor-only means signing up for that work.

An enclosure is not automatically better than indoor-only. A bare wire box with a concrete floor gives a cat fresh air and nothing else. An indoor home with vertical routes, window perches, scheduled play and rotated foraging beats it comfortably.
Building an enclosure that holds
Think about where cats actually get out, in order of frequency.
The roof, or the absence of one.
A cat climbs the corner post, walks the top rail and drops out. Any enclosure without a roof is a temporary arrangement. Solid roofing over part of it also provides the shade and rain shelter the cat needs.
The join between panels and the join to the house.
Cats find the gap where a panel meets brickwork, or where mesh meets a soffit. A cat can pass through a gap it can fit its head and shoulders through. Check the whole perimeter at cat height and above, not at eye level.
The door.
Every enclosure with a single door will eventually be left open. A double-door airlock, so that one door closes before the next opens, removes the failure entirely. If that is impossible, a self-closing spring and a habit of latching before you turn around is the minimum.
Underneath.
Cats dig less than dogs but will exploit a soft gap. Bury the mesh, run it outward as an L-shaped skirt under paving, or build onto a solid base.
Materials.
Welded mesh holds; chicken wire does not, and it also cuts. Soft netting stretches and a determined cat gets teeth into it. Check every fixing after a storm.
Climbing aids you did not intend.
A wheelie bin, a garden chair, a low shed roof or a maturing shrub next to the enclosure becomes a launch pad. Re-audit each spring when the planting has grown.

An enclosure in full sun can become a heat trap the cat cannot leave.
Cats have very limited evaporative cooling. They do not sweat usefully and panting in a cat is already an emergency sign, not a cooling mechanism. A mesh run with a clear polycarbonate roof, a paved floor and no shade behaves like a greenhouse. Provide deep shade for the whole hottest part of the day, always leave water inside the enclosure, and give the cat free access back into the house rather than shutting it out. Long-coated cats and flat-faced breeds such as Persians and Exotics reach danger sooner than others.

Making an enclosure worth having
Space alone does not enrich. Complexity does.
Height.
Cats resolve their own tension by going up. Shelves and ramps at two or three levels turn a small footprint into a usable territory.
Cover.
A cat that feels exposed will not use a space. Hides, planting and a solid section of wall on at least one side matter more than square metres.
Substrate variety.
Grass, bark, a log, a sunny paving slab. Different surfaces give different sensory information and different scratching options.
Scent.
Let the enclosure get rained on, let leaves blow in, add a cut branch occasionally. Scent turnover is most of what a cat gets from being outdoors.
Safe planting.
Check every plant before you put it in. Lilies of any kind are absolutely excluded because of the kidney risk to cats.

Moving a free-roaming cat to containment
This is where most attempts fail, because owners stop outdoor access before the alternative exists.
Build first.
The enclosure, the enrichment and the litter provision all need to be in place and used before the cat flap is locked. A cat that finds its territory shut off with nothing offered in exchange will spend a fortnight at the door.
Expect a protest phase.
Yowling at doors, pacing, sitting at windows and night activity are normal for the first weeks. They fade. What extends them is intermittent access, because an occasional escape teaches the cat that persistence works.
Add litter capacity.
A cat that has been toileting outside may not have a strong tray habit. Offer more trays than cats, uncovered, in quiet places, with a fine unscented substrate.
Feed to the new activity level.
Reduce the ration when the roaming stops, and weigh the cat monthly. By the time an owner notices the change by eye, it is substantial.
Watch the litter tray closely for the first month.
A stressed indoor transition is a classic trigger for cystitis. Small frequent visits, straining, blood, or crying in the tray need a vet.
Keep the microchip details current.
This matters more, not less, once the cat is contained, because a contained cat that does get out is disoriented and travels further.
What changes by cat
Deaf cats, and white cats with blue eyes.
Congenital deafness is common in this group. A deaf cat cannot hear a car, a dog or another cat approaching. Containment is not a preference here.
Blind or visually impaired cats.
They navigate a known indoor map well and an outdoor world badly. Keep furniture layout stable.
FIV positive cats.
Containment protects other cats from infection, and protects the FIV positive cat from the infections its own immune system handles poorly. This is the single strongest containment indication.
Senior and arthritic cats.
Reduced escape speed, reduced hearing, and difficulty getting up onto the routes they used to use. Ramps rather than jumps inside the enclosure.
Very active breeds.
Bengals, Abyssinians, Savannahs and similar cats are the ones that defeat weak enclosures and the ones that suffer most from a bare indoor life. Build stronger and enrich harder.
Flat-faced breeds.
Persians and Exotics have compromised airway cooling. Do not leave them in a sunlit enclosure.
Cats that hunt effectively.
Hunting brings in parasites and, in some regions, disease. It also has a real wildlife cost that a full enclosure removes entirely.
Preventive care differs by containment status
This is the part owners most often get wrong after a change. The plan that suited a free-roaming cat is not the plan for a contained one, and the reverse.
What the vet will want to know
Bring the access history. It reframes half the differential list.
What never to do
Do not use a dog flea product on a cat. Permethrin at dog concentrations causes severe, often fatal, neurological toxicity in cats, and it transfers by contact between animals in the same house.
Do not fit a non-breakaway collar, and do not rely on a collar tag instead of a microchip.
Do not stop outdoor access abruptly with nothing offered in exchange, then reopen it because the cat protested. Intermittent access is the one pattern that guarantees a long protest.
Do not treat a walled garden as an enclosure. A wall is a climbing surface.
Do not shut a cat out of the house into an enclosure with no route back in, particularly in heat or cold.
Do not assume a microchip cat flap keeps other cats out for good. It reduces entry, but a determined neighbourhood cat learns to follow a resident cat through, and a flap in an external door is also a route a cat can be locked out of.

When to get help
Get same-day veterinary care for a cat straining in the litter tray without producing urine, for any wound, limp or hot swelling after outdoor access, for suspected poisoning, and for panting or open-mouthed breathing after time in a sunlit enclosure.
Book within the week for weight gain since a change in access, for a cat that has stopped using the litter tray, for increased drinking, and for overgrooming or bald patches that appear after a lifestyle change.
If your cat is missing, search sheds, garages and outbuildings within a few houses first, at night and with a torch, and contact your microchip database to flag the chip as missing. Most cats found alive are found close to home and shut inside something.
