Why a wound is different underwater

On land, a scrape dries, scabs and heals. Underwater, a wound sits permanently bathed in a soup of bacteria, fungi and parasites, so the same injury behaves very differently. A fish, amphibian or invertebrate has no chance to form a dry scab. Its defence is the slime coat, a thin layer of mucus over the skin, plus the epidermis and dermis beneath it where scales are anchored.

That mucus does far more than make a fish slippery. It is a physical barrier against pathogens, it carries antibodies and enzymes that attack invaders, it smooths locomotion, and it helps manage osmosis so the animal does not flood with or lose water. A healthy fish shows an even, glossy sheen and uniform scales. The moment that coat is stripped, cracked or clouded, the barrier is open and infection can move fast. This is why handling, netting and even sudden water changes matter so much: damage the coat and you have created a wound before any decor ever did.

Reading your fish every day

Early detection is everything, because an ulcer that is trivial on day one can be systemic by day five. Spend a few minutes daily watching behaviour and body.

Healthy animals are active, responsive and unblemished. Warning changes fall into two groups. Physical signs include missing scales, open sores or ulcers, red streaks or patches (erythema), frayed or torn fins, cotton-like tufts, and a slime coat that looks thick, cloudy or is sloughing away. Behavioural signs include lethargy, hiding, loss of appetite, clamped fins, and flashing, the darting rub against decor or substrate that says something is irritating the skin or gills.

Most trouble starts as physical trauma, then a secondary infection colonises the open tissue. Knowing which is which decides the treatment, because a fungal medication does nothing to a bacterial infection and vice versa.

Build the kit before you need it

Step by step: treating a wounded fish

  1. Isolate the patient. Move the injured animal to the pre-cycled hospital tank. This stops disease spreading, removes bullying, and lets you medicate without dosing your whole display or its invertebrates and plants.
  2. Lock down water quality. In the hospital tank, ammonia and nitrite must read 0 ppm. Do small daily water changes of 10–25% to keep the pathogen load low. Clean water is itself a powerful medicine and often the only one a minor wound needs.
  3. Start supportive care. If the species tolerates it, add aquarium salt at about 1 tablespoon per 11–19 L (3–5 gal). Salt reduces osmotic stress so the fish spends its energy on healing, and it has a mild antiseptic effect.
  4. Identify, then medicate. Match the drug to the sign: antifungal for cottony growth, antibacterial for ragged fins or ulcers. Follow the label dose exactly and never combine medications unless a vet directs it.
  5. Monitor and finish the course. Watch daily for wound closure, fin regrowth and returning appetite. Complete the full recommended duration even if the fish looks better early, or the survivors of the infection simply rebound.

Salt, medicated baths and the different treatment camps

Aquarists genuinely disagree on approach, and each camp suits different keepers and species.

The clean-water minimalists rely on daily changes and salt alone, adding no drugs unless a clear infection sets in. It is gentle, cheap and safe for invertebrates, and it suits community and planted tanks. The targeted-medication approach reaches for a species-specific antibacterial or antifungal as soon as a diagnosis is clear; it acts faster on established infections but stresses the fish and can crash a biofilter. The medicated-bath or dip method places the fish briefly in a stronger concentration in a separate container, which can clear stubborn external infections that a whole-tank dose cannot, at the cost of extra handling stress. Finally, veterinary intervention covers topical antiseptics or antibiotics applied directly to a wound, sometimes under light sedation, for deep or non-healing injuries.

Regional habits differ too. In the United States, hobby antibiotics have historically been sold over the counter, while in the United Kingdom, the European Union, Australia and increasingly elsewhere many antibacterials are prescription-only, so keepers there lean harder on water quality, salt and licensed remedies. Neither route excuses guessing; misusing antibiotics breeds resistance everywhere.

Prevention is the real cure

The best wound is the one that never opens. Excellent, stable water quality is the foundation: regular changes, filtration sized to the stock, and no overcrowding. Reduce conflict with compatible tank mates and generous hiding spots, and check every new ornament, rock or plastic plant for edges sharp enough to tear a fin. A varied, high-quality diet builds the immune system that fights infection from the inside. And the golden rule, the one that prevents most disaster outbreaks, is to quarantine every new arrival for 4–6 weeks before it ever meets your main tank.

Keep a simple log of water parameters, feeding response and any change in appearance. Weekly photos make slow trends obvious that daily glances miss.

Age, species and the scaleless exception

One dose does not fit all. Fry and juveniles are fragile and often cannot handle standard medication strengths. Senior fish carry weaker immune systems and heal more slowly. Long-finned fish such as bettas and fancy goldfish tear and rot fins easily. And scaleless fish, including kuhli loaches and many catfish, absorb salt and medications far more readily, so they need reduced doses or entirely different treatments.

Only if the entire population is affected and the medication is confirmed safe for every inhabitant, including invertebrates and plants. Most fish medications harm shrimp, snails and the biofilter, so a separate hospital tank is almost always the safer choice.
Is aquarium salt safe for every fish?
No. Salt helps many species by easing osmotic stress, but scaleless fish such as loaches and some catfish, and various soft-water species, are sensitive. Research your species and start low, or skip salt and rely on water quality.
How long before I should expect improvement?
Minor fin tears and single lost scales often improve within days on clean water alone. For treated infections, look for a positive trend within 5–7 days. No improvement or worsening in that window means you should reassess the diagnosis, recheck water parameters, and contact an aquatic vet.
A wound appeared right after I added new decor. What now?
Remove or sand down the culprit. Sharp edges on rock, resin ornaments and hard plastic plants are a common cause of torn fins. Then keep the water pristine and let the fish heal.
